摘 要:目的:与传统10g尼龙丝记录法及神经肌电图检测中神经传导速度检测比较,探讨改良10g尼龙丝记录法(即与足面感觉对比记录法)对查糖尿病周围神经病变的有效性。方法:设神经传导速度检测结果特异性为100.0%,分别采用改良10g尼龙丝记录法(改良法)及传统10g尼龙丝记录法(传统法)检查88例2型糖尿病患者,通过比较改良法及传统法在周围神经病变诊断方面的敏感性及特异性。结果:与神经传导速度比较,改良法与传统法的敏感性分别为(94.4%,20.4%),特异性分别为(85.3%,76.5%),与神经传导速度检测的Kappa值分别为(0.806,-0.027),提示改良法在敏感性及特异性方面均高于传统法,差异有统计学意义(P<0.01)。结论:改良10g尼龙丝记录法可明显提高10g尼龙丝检测方法的敏感性与特异性。Objective:To investigate the effects of improved 10g nylon wire recording method ( feet and face sensory compared recording method) on the detection of diabetic peripheral neuropathy. And to compare it with conventional 10g nylon wire recording method and the nerve conduction velocity detection of neural EMG detection. Methods:The nerve conduction velocity was set as 100.0%. 88 type 2 diabetic patients were respectively detected by improved 10g nylon wire recording method ( improved method ) and conventional 10g nylon wire recording method (conventional method). Results: Compared to the nerve conduction velocity, the sensibility of improved method and conventional method were 94. 4% and 20. 4% respectively, the specificity were 85. 3% and 76. 5%respectively, and the Kappa value with nerve conduction velocity detection were 0.806 and -0.027 respectively. It showed that all the aspects of the sensibility and specificity of the improved method were higher than the conventional method, which had statistical significance ( P〈0.01) . Conclusion:Improved 10g nylon wire recording method can significantly improve the sensibility and specificity of 10g nylon wire recording method.
