血清cystatin C在2型糖尿病肾病早期检测中的应用价值

摘　　要：目的分析血清cystatin C对2型糖尿病肾病患者的诊断价值。方法收集2型糖尿病患者160例,对照组为50例健康体检者,采用乳胶颗粒增强免疫比浊法测定血清cystatin C浓度,并进行分析。结果 1血清cystatin C浓度在微量白蛋白尿组、大量蛋白尿组均较对照组高,差异有统计学意义(P<0.05);2血清cystatin C随着年龄的增高其平均水平有上升的趋势,且在60岁以后增高有统计学意义;3以对照组的(+2s)为阳性界值,血清cystatin C阳性率均高于同组BUN和Scr阳性率(P<0.05)。4血清cystatin C、Scr与UAER成正相关。结论血清cystatin C是一个较敏感的反映早期糖尿病肾病肾小球损伤的指标。联合检测UAER和血清cystatin C有利于早期发现糖尿病肾损害,并进行早期干预。[ Objective ] To analyze the diagnostic value of cystatin C in type 2 diabetic patients. [Methods] A total of 160 diabetic patients were enrolled, the control group contained 50 normal persons （male/female=24/26）. Serum cystadn C was determined by Particle-enhanced turbidimetric immunoassay, and the data were analyzed by the software. [ Results ] Serum cystatin C concentration increased significantly in patients with microalbuminuira and macroalbumLnuira（all P 〈0.05）. The mean value of the serum cystatin C increased with age increased, and there was significantly statistical difference above 60 years. In the control group （＋2s） positive for boundary value, the positive rate of serum cystatin C was significantly higher than that of BUN and Scr in the same groups （P 〈0.05）. A positive correlation was noted （Correlation coefficients with UAER were 0.403 for serum cystatin C, 0.237 for serum creati- nine, P all〈0.001）. [Conclusions] Serum cystatin C is a more sensitive maker that reflects glomerular injury in early diabetic nephropathy. To detect UAER and serum cystatin C will help us identify the subjects with a slight reduction of renal function, and observe the development of stage in diabetic nephropathy in order to offer early intervention.
