控制性超排卵中提前发育卵泡处理方案研究

摘　　要：目的研究长方案控制性超排卵过程中出现提前发育卵泡是否影响体外受精-胚胎移植结局;穿刺提前发育卵泡对助孕结局有无影响。方法回顾性研究长方案超排卵过程中出现提前发育卵泡的80例患者及卵泡发育同步的80例患者。根据卵泡发育情况及处理方法分为3组:A组,穿刺提前发育卵泡组(42例);B组,不穿刺提前发育卵泡、Gn原量治疗组(38例);C组,卵泡发育同步组(C组)。比较3组患者HCG日血P、LH水平、Gn用量、获卵数、成熟卵率、受精率、优质胚胎率、临床妊娠率及流产率。结果 3组患者的HCG日血P、LH水平、Gn量、获卵数、成熟卵率、受精率、流产率均差异无统计学意义;C组优质胚胎率、临床妊娠率较A、B组有增高趋势,但差异无统计学意义。结论长方案控制性超排卵中出现提前发育卵泡时并不影响助孕结局,穿刺提前发育卵泡与不穿刺提前发育卵泡获得的妊娠结局相同。[Objective] To study whether there is influence on the IVF-ET outcome when the follicles developed in advance in controlled ovarian hyperstimulation （COH） with long protocol; Whether aspirating the follicles devel- oped in advance impact the chnical outcome. [ Methods ] Retrospectively analyzed 80 patients whose follicles devel- oped in advance stage and 80 patients whose follicles developed synchronously when treated in COH with long pro- tocol .They were divided into 3 groups in according to follicles developing condition and different treatment methods： Group A （n =42）, aspirating the follicles developed in advance; Group B （n =38）： no aspiration when the follicles de- veloped in advance, treated with Gn as before; Group C （n =80）, follicles developed synchronously. The indexes in- cluding the level of P and LH in the day of HCG administration （day-of-HCG）, the total gonadotropin dosage, the number of retrieved oocytes, the proportions of mature oocytes, the fertilization rate, the good quality embryo rate, clinical pregnancy rate in the three groups were compared. [Results] Among three groups, there was no significant difference in the level of P and LH in the day-of-HCG, the total gonadotropin dosage, the number of retrieved oocytes, proportion of mature oocytes and the fertilization rate. When compared group A and B with group C, there was rise trend in the good quality embryo rate and clinical pregnancy rate, but there is no significant difference. [ Conclusions] There is no influence on the pregnancy outcome when the follicles developed in advance in con- trolled ovarian hyperstimulation （COH） with long protocol; Aspirating the follicles developed in advance get the same pregnancy outcome as group without aspiration.
