路径管理护理在垂体腺瘤手术患者护理中的应用效果

摘　　要：目的探讨路径管理护理在垂体腺瘤手术患者护理中的应用价值。方法 2010年1月至2012年1月间收治的76例垂体腺瘤手术患者作为对照组,采用常规管理护理;2012年1月至2014年1月间收治的77例垂体腺瘤手术患者作为研究组,采用路径管理护理。比较两组患者的经济效果、护理质量和临床效果。结果研究组总住院时间为(14.80±2.21)d,对照组为(19.12+1.19)d,差异有统计学意义(t=26.514,P<0.001)。研究组住院费用为(20534.8±10.12)元,对照组为(22213.7±12.01)元,差异有统计学意义(t=2.661,P=0.007)。研究组患者满意度和护理人员满意度分别为100.0%和97.9%,对照组分别为88.2%和94.5%,差异有统计学意义(P<0.05)。两组所有患者均治愈,但研究组术后并发症发生率、再住院率和再手术率分别为2.6%、0和0,对照组分别为21.1%、3.9%和3.9%,差异有统计学意义(P<0.01)。结论路径管理护理在鼻蝶入路垂体腺瘤术患者护理中具有一定的可行性和实用性,可有效缩短住院时间,降低住院费用,提高患者和护理人员满意度,并明显降低术后并发症发生率。Objective To investigate the value of pathway management transsphenoidal pituitaryadenoma resection.Methods 76 patients received from January 2010 to January 2012 as control group was nursed conventional management,77 patients received from January 2012 to January 2014 as study group was nursed pathway management.The economic consequence,care quality and clinical outcome were compared.Results The total lengths of hospital stay in study group were（14.80 ± 2.21） d,significantly lower than the control group（19.12 ± 1.19） d（t = 26.514,P〈0.001）,research group＇ s hospitalization costs was（20534.8 ± 10.12） yuan,significantly lower than control group（22213.7 ± 12.01） yuan（t = 2.661,P = 0.007）;the satisfaction of patients and caregivers in control group was 100.0% and 97.9%,significantly higher than control group（88.2% and 94.5%）（P〈0.05）;all patients were cured,but complications rate,readmission rates and reoperation rate was 2.6%,respectively,0 and 0,significantly lower than control group（21.1%,3.9% and 3.9%）（P〈0.01）.Conclusion The path managed care in transsphenoidal resection of pituitary adenoma patient care is feasible and practical,which can effectively shorten hospital stays,reduce hospital costs and improve patient and caregiver satisfaction,and significantly reduce complications rate.
