Treatment of Garden Ⅲ and Ⅳ femoral neck fracture by using small cuts in front of the hip and hollow screw internal fixation in young adults

摘  要:目的探讨采用前方小切口切开复位联合经皮空心拉力螺钉固定治疗青壮年Gardenm、Ⅳ型股骨颈骨折的方法及疗效。方法2008年2月—2013年8月对68例股骨颈骨折采用前方小切口切开复位联合经皮三枚空心拉力螺钉固定治疗,其中男45例,女23例,年龄(37.5±5.2)岁,按Garden分型,Ⅲ型45例,Ⅳ型23例,均于伤后3d内施术。结果所有患者均获得GardenⅠ、Ⅱ级复位,经随访12~60个月,无不愈合及股骨头坏死发生,末次随访Harris评分优55例,良13例,优良率100%。结论采用前方小切口切开复位联合经皮空心拉力螺钉固定治疗青壮年Gardenm、Ⅳ型股骨颈骨折手术简便,创伤小,愈合率高,疗效良好,是一种理想的微创方法。Objective To discuss the clinically therapeutic effects of the small cuts in front of the hip and hollow screw internal fix- ation on femoral neck fractures ( Garden Ⅲ and Ⅳ) among young patients. Methods From Feb. 2008 to Aug. 2013, the combi- nation of small cuts in front of the hip and hollow screw was applied to the treatment of 68 young patients with femoral neck fractures E 45 males, 23 females ; aged (37.5 ± 5.2) years ]. According to Garden classification, the fractures consisted of type Ⅲ in 45 cases and Ⅳ in 23 cases. Operation was completed within 3 days after injury. Results The reduction of all the patients reached Garden Ⅰ or Ⅱ. During the follow-up of 12 to 60 months, neither nonunion nor femoral avascular necrosis happened. At the last time of the follow-up, the clinical results were assessed according to the Harris scores: excellent in 55 and good in 13 cases, with an excellent and good rate of 100%. Conclusion The application of small cuts in front of the hip and hollow screw internal fixation to treating femoral neck fracture ( including Garden Ⅱ and Ⅳ ) among young patients has the advantages of simple operation, little trauma, high healing rate of fractures and good curative effect. It is an ideal minimally invasive treatment method.
