健脾清肠方对脾虚湿热型激素依赖溃疡性结肠炎患者GRα表达的影响

摘　　要：[目的]探讨健脾清肠方治疗脾虚湿热型激素依赖溃疡性结肠炎的作用机制。[方法]选取脾虚湿热型激素依赖溃疡性结肠炎患者60例,随机数字表法分为试验组和对照组各30例。2组均给予强的松基础治疗,试验组加服健脾清肠方,对照组给予补脾益肠丸。疗程为12周。患者采用标准激素减量方法撤减激素。2组于治疗前后分别进行中医证候疗效及激素撤退情况分析。使用免疫细胞化学染色方法检测患者外周血单个核细胞(PBMC)中GRα的表达,比较治疗前后变化情况。[结果]试验组中医证候疗效总有效率为96.7%,对照组为80%,试验组中医证候改善明显优于对照组(P<0.01);2组治疗后累积光密度值均明显升高(P<0.01),试验组上调GRα表达水平优于对照组(P<0.01)。[结论]与对照组比较,健脾清肠方可有效改善脾虚湿热型激素依赖溃疡性结肠炎的临床中医证候,上调GRα的表达水平,改善激素依赖状况,具有良好的临床疗效。[Objective]To explore the mechanism of Jianpiqingchang formula in treatment on steroid-dependent ulcerative colitis（SDUC）patients with TCM type of spleen-deficiency and retention of damp-heat.[Methods]Sixty SDUC patients with TCM type of spleen-deficiency and retention of damp-heat were selected and randomly divided into two groups：the trial group and the control group,the trial group treated with Jianpiqingchang formula and the control group treated with Bupiyichang pill.The course is 12weeks.Comprehensive clinical effect in each group before and after the treatment,as well as hormone with drawal were analyzed.In addition,the expression of GRα of PBMC in patients was determined by immunocytochemical method.[Results] As to the efficacy of TCM symptoms,the total effective rate of the trial group is more effective than the control group（P〈0.01）,which is 96.7% and 80%,respectively.The expression level of GRα in trial group is higher than that in the control group after treatment（P〈0.01）.[Conclusion] Compared with the control group,Jianpiqingchang formula can effectively improve clinical syndromes of SDUC patients with TCM type of spleen-deficiency and retention of damp-heat,and up-regulate the expression of GRα so as to improve hormone dependent status.
