摘 要:目的分析四川省遂宁市自然人群中乙型肝炎(乙肝)病毒(Hepatitis B Virus,HBV)主要基因型S基因主要亲水区[Major Hydrophilic Region,MHR;氨基酸(Amino Acid,aa)99-169]及人类白细胞抗原I型(The Putative Human Leukocyte Antigen Class I-Restricted,HLA-I)细胞毒性T淋巴细胞(Cytotoxic T Lymphocyte,CTL)表位aa位点置换特点。方法遂宁市乙肝血清流行病学调查中乙肝病毒表面抗原阳性人群,扩增HBV S基因片段并测序得到的完整S基因片段序列,进行生物信息学比对分析。结果在遂宁市共得到108份序列,其中B基因型40份,C基因型68份,在S基因MHR发生aa置换位点为Y100C、Q101H、L110F、T126A/S、T131I、S136F、T143M、G145R、V168A,在HLA-I型CTL表位aa置换位点为V47A、P46L、T47K、L88P、I92T、L98V、Y100C、Q101H、V177A、V194A,位点置换在年龄分布和基因型分布的差异无统计学意义。结论遂宁市HBV S基因MHR及HLA-I型CTL表位aa位点置换与基因型分布无关,未发现接种乙肝疫苗与人群中HBV S区aa位点置换相关。Objective To determine the frequency of amino acid (aa) substitutions in the major hydrophilic region (MHR;aa99- 169) and the putative human leukocyte antigen (HLA) class I-restricted cytotoxic T lymphocyte (CTL) epitope region of the S gene, in the dominate genotype of hepatitis B virus (HBV) in Suining city, Sichuan province of China. Method We collected serum samples from HBV surface antigen (HBsAg) positive cases during a sero-epidemiology survey in Suining city. The complete S gene of HBV was amplified and sequenced. The aa sequences were analyzed with bioinformatics software. Result There were 108 sequences; 40 were genotype B and 68 were genotype C. The aa substitutions in the MHR of S gene were Y100C, Q101H, LIlOF, T126A/S, T131I, Si36F, T143M, G145R, V168A, and in the CTL epitope region of the S gene were V47A, P46L T47K, L88P, I92T, L98V, Y100C, Q101H, V177A, and V194A. There were no significant differences in aa substitutions in different genotypes and different age groups. Conclusion In Suining city, aa substitution in the MHR and HLA CTL of HBV S gene showed little difference betweengenotype B and C. No evidence was observed that showed aa substitution of S gene being related to the hepatitis B immunization program.
