两种常用方式治疗严重胫腓骨远端粉碎性骨折的疗效分析

摘　　要：【摘要】目的对有限内固定结合外固定支架和锁定钢板两种常用手术治疗严重胫腓骨远端粉碎性骨折的疗效进行分析。方法回顾分析本院2010年1月至2013年12月获得随访的256例严重胫腓骨远端粉碎性骨折患者的两种手术治疗方式疗效，按手术治疗方式不同分组：A组135例接受有限内固定结合外固定支架治疗，B组121例接受锁定钢板治疗。比较两组的术前住院时间、手术时间、术后住院时间、术后感染率，同时对患者术后功能进行评分。结果A和B组手术时间和术后住院时间统计结果基本相似：A组术前住院时间和术后感染率均低于B组，两组差异有统计学意义（P〈0．05）。手术后随访12个月，有限内固定结合外固定支架的优良率是96．3％，锁定钢板的优良率90．9％，两组差异有统计学意义（P〈0．05）。结论在手术治疗严重胫腓骨远端粉碎性骨折中，有限内固定结合外固定支架比锁定钢板更具有优势，是较为理想的手术治疗方法。Objective The therapeutic effect analysis of serious open comminuted fracture of distal tibia and fibula is analyzed for two common therapeutic methods including limited internal fixation with external fixation apparatus and lock plates operations. Methods The curative ef- fects of follow-up visited 256 patients of serious open comminuted fracture of distal tibia and fibula are reviewed. The 256 patients are classified into two groups A and B according to operation method. Group A includes the 135 patients with limited internal fixation with external fixation apparatus operation, group B the 121 patients with lock plates operations. The perioperative indicators include length of preoperative hospitliztion, length of operation, length of operation postoperative hospidiztion, postoperative infection rate and functional recovery grade are compared. Results Group A is of shorter length of preoperative hospitliztion and postoperative infection rates than Group B, and 96.3% good rate of Group A is higher than 90.9% of Group B after 12 months since operation. Differences between the two groups have statistical significance （P〈0.05）. Conclusion limited internal fixation with external fixation apparatus operations is the better therapeutic methods for patients of serious open comminuted fracture of distal tibia and fibula.
