摘　　要：目的分析Madelung病的临床特征并探讨其治疗方法。方法结合文献分析1例Madelung病患者的临床表现、CT影像检查、诊断、治疗及预后情况。结果患者主要表现为身体上半部脂肪弥漫性沉积,颈部畸形和活动受限,有饮酒嗜好,并伴有多种内科疾病,如酒精性肝病、高血压病、高尿酸血症、甲状腺功能减退等。经手术治疗和内科治疗后,现恢复良好,无复发及恶变倾向。结论 Madelung病具有特征性CT表现,即颈项部、下枕部、上背部皮下有大量脂肪组织蓄积增厚,其密度和信号强度与正常脂肪相同。本病可采用综合治疗的方法 ,除戒酒外,可将吸脂术与外科手术相结合,同时积极治疗内科疾病,这样有利于美观、减少复发及并发症,且改善预后。Objective To analyze the clinical characteristics and explore the treatment method of Madelung＇ s disease. Methods The analysis was made on the clinical manifestations, CT imaging examination, diagnosis, treatment, and prognosis of 1 patient with Madelung＇ s disease, along with literature review. Results The manifestations of the patient were diffuse fatty mass in the upper body, neck deformity, restricted motion, drinking habit, and several internal medicine diseases, such as alcoholic liver disease, hypertension, hyperuricemia, and hypothyroidism. After surgery and medical treatment, the patient recovered well without recurrence and malignant transformation. Conclusion The CT imaging of Madelung disease shows the feature of accumulated fat deposited at subcutaneous sites of the anterior or posterior of the neck, the lower occipitalia and the upper back. The density and signal intensity of the excess fat are similar as normal fat. Comprehensive treatments can he applied for the treatment including temperance, combination liposuction and lipectomy, and active medical therapy. That can benefit patient＇ s outlook, decrease the recurrence and complications, and get favourable prognosis.
