外周血干细胞冠脉内移植治疗心梗后合并心功能不全的临床价值研究

摘　　要：目的研究并分析采用外周血干细胞冠脉内移植进行治疗心梗后合并心功能不全的临床效果及其意义。方法在我院选取自2013年6月至2014年3月心梗后合并心功能不全的患者60例,所有患者都进行PCI治疗,并同时随机分为采用干细胞移植治疗的观察组和采用传统的治疗方法的对照组两组各30例。观察组患者在常规治疗冠心病的基础上再采用粒细胞集落刺激因子进行皮下注射刺激患者自身体内的骨髓干细胞,第六天时进行分离患者的外周血干细胞悬液,然后再按照要求将收集的干细胞悬液经过导管注入梗死的冠脉后再进行移植;而对照组患者则采用常规的治疗方法进行治疗。移植前后半年之内采用超声心动图进行评价室壁节段运动指数和左室的形态以及左室心功能指标,对比观察两组冠心病患者在治疗后的生存率以及出现心脏事件的机率。结果观察组患者经治疗后较治疗前其心脏收缩以及舒张末容积均显著减少,左室射血分数明显增高,左室壁节段运动指数显著降低,治疗前后的差异具有统计学意义;而且对照组患者与观察组患者比较术后上述的指标发现差异具有统计学意义,而两组患者在术后的生存率以及心脏事件发生率不存在差异。结论采用外周血干细胞冠脉内移植进行治疗心梗后合并心功能不全的效果较佳,可较快地改善心功能不良的状况。Objective To study and analyze the use of intravascular peripheral blood stem cells transplantation for treatment of the clinical effect of combined cardiac function after myocardial infarction and its significance. Methods 60 patients with selected in our hospital, all patients were treated for PCI, within half a year before and after transplantation with echocardiography PingJiaShi segmental wall ＇motion index and left ventricular morphology and left ventricular cardiac function indexes, compared to observe the survival rates of two groups after treatment in patients with coronary heart disease and the risk of heart attacks. Results The observation group of patients after treatment than before treatment the cardiac systolic and diastolic volume were significantly reduced, at the end of the left ventricular ejection fraction increased obviously, the left ventricular segmental wall motion index significantly reduced; statistically significant differences between before and after the treatment. Conclusion Use of intravascular peripheral blood stem cells transplantation for treatment of myocardial infarction （ ami ） after the merger of cardiac insufficiency effect is better, can quickly improve cardiac function in bad condition.
