广西钟山县艾滋病治疗“一站式服务”模式的效果和存在问题

摘　　要：目的探讨广西钟山县艾滋病治疗"一站式服务"模式的效果和存在的问题,为艾滋病感染者和病人规范治疗、科学管理提供科学依据。方法通过对艾滋病患者的"积极治疗",实施"一站式服务"模式,即在加快初筛、确认、咨询、体检及抗病毒治疗等一系列工作的情况下,对所有确认阳性的艾滋病患者提供抗病毒治疗咨询,不管CD4淋巴细胞数的多少,如果患者愿意且医生认为应该给予治疗的,提供抗病毒治疗;对既往已报告但尚未接受国家免费艾滋病抗病毒治疗的艾滋病患者提供抗病毒治疗。结果新发病例抗病毒治疗覆盖率从实施前(2011年)的70.04%(194/277)提高至实施后(2013年)的95.18%(395/415)(χ2=82.903,P<0.01);新发现病例病死率从实施前的23.92%(50/209)降低到2013年的4.06%(28/689)(χ2=79.740,P<0.01);艾滋病示范区综合管理指标得到明显提高。结论艾滋病治疗"一站式服务"模式为病人提供了综合服务平台,使各项检测项目能够在治疗点得以及时完成。此模式具有适宜性和可行性,此经验可在广西及至全国各地推广。OBJECTIVE To explore the effect and problem of HIV/AIDS treatment of ＂one-stop service model＂in Zhongshan CountyGuangxi,to provide a scientific basis for standard treatment and scientific management targeting to PWLHA. METHODS Through theactive treatment of HIV/AIDS. The implementation of ＂one-stop service＂ model, that is to speed up a series of work, includingHIV screening, confirmatory test, counseling, physical examination and ART, providing ART consultation to all of those withHIV-WB positive,regardless of the number of CD4 lymphocytes,and providing ART for those who willing to have and doctors be-lieve that should have;provide antiviral therapy on the previous reported but not receive free AIDS antiretroviral treatment of AIDSpatients. RESULTS The newly discovered antiviral treatment coverage from the patient before the implementation of the 70.04%（194/277） in 2013 increased to 95.18%（395/415）（χ^2=82.903,P〈0.01）;new found a case fatality ratio from before the implementation ofthe 23.92%（50/209） decreased to 4.06% in 2013（28/689）（χ^2=79.740,P〈0.01）;integrated management index demonstration AIDShas been improved obviously. CONCLUSION The ART with＂one-stop service＂ model provides patients with a comprehensive ser-vice platform that enables the test items completed timely in ART site. This mode has the suitability and feasibility and this experi-ence can be promoted in Guangxi even all over the country.
