某部队人群肺结核患病情况的调查

摘　　要：目的 :了解南方部队结核病的发病现状和流行特征 ,为制定部队结核病防治措施提供依据。方法 :以整群抽样方式 ,抽取我国驻南方地区 A、 B两个部队为观察对象 ,于 1997年 1月至 2 0 0 1年 1月 ,收集相关的结核病患病资料 ,现场应用结核菌素试验了解官兵结核感染情况。应用 L ogistic回归模型分析影响发病的危险因素。结果 :在 4年观察期间 ,两部队共发现活动性肺结核病人 71例 ,年均患病率为 179.99/10万 ,各年度患病率分别为 2 6 4 .2 8/10万 ,173.4 7/10万 ,16 4 .36 /10万和 119.19/10万。趋势检验差异有显著性。战士发病多于干部 ,新兵多于老兵 ,发病主要集中在 2～ 4月。各年度结核菌素阳性率差异无显著性 ;多因素分析中 ,与发病有显著性关系的危险因素为结核病接触史、人际交往、个人卫生习惯、卡介苗接种史。结论 :南方部队结核病患病率呈缓慢下降趋势 ;官兵患结核病与个人有结核病接触史、人际交往活跃、个人卫生习惯差、吸烟和卡介苗接种史有关。Objective:To acknowledge TB infection situation and its epidemic characteristics in southern army and to provide the effective prevention and treatment methods for TB.Methods:Using the method of cluster sampling,we sampled team A and team B in a southern army.The TB infection history and current infection situation by PPD test were collected from 1997 to 2001,logistic regression model was applied to analyze the risk factors for TB infection.Results:During a 4 years period of observation,the number of TB actively infected patients was 71,the average prevalence was 179 99/10 5,and the respective rate was 264 28,173 47,164 36,and 119 19/10 5 for each year,respectively.Prevalence in soldiers was higher than that of cadres,and junior soldiers were higher than senior ones.The infection time was more concentrated on february to april.The positive rate of PPD test was no significant difference among each year.Based on the results of multi factors analysis,the significant risk factors included the contacting history of the TB patients,social relationship,personal hygient habits,smonking the injection history of BCG vaccine.Conclusion:The TB prevalence is gradually decreased in southern army,TB infection is related to TB patients contacting history,active in social relationship,a bad personal hygient habits,smoking and BCG vaccine injection.
