泽芪汤对肝硬化门静脉高压症患者血流动力学及肝纤维化指标的影响

摘　　要：目的观察泽芪汤对肝硬化门静脉高压症患者血流动力学和肝纤维化指标的影响。方法将60例肝硬化门静脉高压症患者随机分2组。2组均常规接受护肝、降酶等对症治疗,治疗组30例加服泽芪汤,对照组30例加服普萘洛尔。2组均治疗3个月后观察治疗前后肝纤维化指标透明质酸(HA)、Ⅲ型前胶原(PCⅢ)、Ⅳ型胶原(Ⅳ-C)、层粘连蛋白(LN)变化,并检测门静脉内径(PVD)、脾静脉内径(SVD)、门静脉血流速度(Spv)及门静脉血流量(Qpv)。结果治疗组治疗后HA、PCⅢ、Ⅳ-C、LN均下降(P<0.05),PVD、SVD、Spv及Qpv均改善(P<0.05);2组治疗后HA、PCⅢ、Ⅳ-C、LN、PVD、SVD、Spv及Qpv比较差异均有统计学意义(P<0.05)。结论泽芪汤能较好地改善肝硬化门静脉高压,降低纤维化指标。Objective To observe the effect of Zeqi decoction on hemodynamics and liver fibrosis indexes in liver cirrhosis patients with portal hypertension. Methods 60 cirrhotic patients with portal hypertension were randomly divided into treatment group and control group. Patients in two groups received conventional protecting liver, reducing enzyme and other symptomatic treatment;the patients in the treatment group were added to Zeqi decoction, pa- tients of the control group received oral propranolol. The course was three months in the two groups. Liver fibrosis markers hyaluronic acid （HA） , procollagen type Ⅲ （PC Ⅲ ） , collagen type Ⅳ （Ⅳ -C） , laminin （LN） , and detect the diameters of the portal vein （PVD）, splenic vein （SVD）, portal vein flow velocity （Spy）, portal venous blood flow （Qpv） were measured before and after treatment. Results After treatment, HA, PC IU, IV - C, LN were decreased in treatment group （ P 〈 0.05 ）, portal vein diameter, blood flow velocity, blood flow before and after treatment were significantly different （ P 〈 0.05 ）, compared with the control group after treatment also showed a significant difference （ P 〈 0.05 ）. Conclusion Zeqi decoction can improve the liver cirrhosis with portal hypertension, decrease the fibrosis index. Curative effect is better than that of propranolol, is an effective prescription in treating liver cirrhosis and portal hypertension.
