摘 要:目的 明确尿微量白蛋白影响因素及其相关性分析。方法 采用随机整群抽样方法,留取崇明县城桥镇40-70岁3159名常住居民的晨尿,测定尿微量白蛋白与尿肌酐,进行体格检查、葡萄糖耐量试验、血脂等生化检测,了解崇明县尿微量白蛋白的发生率,并与各影响因素进行相关性分析,明确尿微量白蛋白的影响因素。结果13159名居民完成调查,占崇明地区人口的83.92/万,平均年龄(55.62±7.99)岁,其中男性925例,女性2234例;23159名居民中,261例尿微量白蛋白与尿肌酐的比值≥20 mg/g,占被调查人数的8.26%;3按尿微量白蛋白与肌酐比值≥20 mg/g为标准,分临床尿微量白蛋白组和正常组,前组血糖、糖化血红蛋白、血脂、血压的均值明显高于后组,差异存在统计学意义(P〈0.05);4应用多元逐步回归法进行分析,结果显示对尿微量白蛋白有影响的因素为糖化血红蛋白、收缩压、总胆固醇。结论 尿微量白蛋白的发生与血糖、血压、血脂密切相关,控糖、调脂、降压是减少尿微量白蛋白的有效手段。Objective To investigate the influencing factors of urine microalbumin and the correlation analysis.Methods A cross-sectional survey with multiple stage stratified cluster and random sampling was performed. All 3159 residents aged from 40 to 70 were involved in the survey. The morning urine was collected to measure urine micro albumin and urine creatinine. Physical examination,75 g oral glucose tolerance test and blood lipid were undertaken. The incidence of urinary albumin in Chongming County the correlation analysis of factors affecting were investigated to clear influencing factors of urine microalbumin. Results ①A total of 3159 residents participated the survey,which was 83. 92 per million of the residents. Of the 3159 residents surveyed,925 were males and 2234 were females. The mean age of the participants was( 55. 62 ± 7. 99) years ranged from 40 to 70 years. ②There were 261 residents( 8. 26%) own the ratio≥20 mg / g. ③According to urine albumin and creatinine ratio≥20 mg / g as the standard,the residents were divided into high microalbuminuria group and normal urinary albumin group,the levels of blood glucose,glycosylated hemoglobin,blood lipid and blood pressure of the former group were significantly higher than the mean group( P〈0. 05). ④Logistic regression analysis demonstrated that the urinary micro albumin was positively correlated with Hb A1 c,SBP and TC. Conclusion The levels of blood glucose,blood pressure and blood lipids were associated with urinary albumin. To control the sugar,lipid and blood pressure is the effective means of reducing urinary albumin.
