动态监测抗凝血酶Ⅲ和D-二聚体在急性心肌梗死诊疗中的应用

摘　　要：目的通过动态监测抗凝血酶Ⅲ(AT-Ⅲ)和D-二聚体(D-dimer)的水平变化,以评价其在急性心肌梗死(AMI)诊断、治疗及预后判断的应用价值。方法检测160例AMI患者(将80例成功实施PCI的AMI患者为PCI组;将80例采用药物保守治疗的AMI患者为药物保守治疗组)及80例健康体检者血浆AT-Ⅲ、D-dimer水平。结果PCI组和保守治疗组治疗前AT-Ⅲ、D-dimer水平与健康对照组相比有显著性差异(P<0.05);PCI组和保守治疗组在治疗后3d、7d、14d的AT-Ⅲ、D-dime水平与治疗后12h相比均有显著性差异(P<0.05);PCI组治疗后12h的D-dime水平与治疗前相比有显著性差异(P<0.05)。结论通过连续监测AMI治疗期间的AT-Ⅲ和D-dimer水平,有助于指导AMI的治疗和预后判断,降低PCI治疗后围手术期并发症的风险。Objective Through dynamic monitoring of AT-Ⅲ and D-dimer level changes,to evaluate the AMI in diagnosis,treatment and prognostic value of application.Methods Detection of 160 cases of AMI patients（80 cases of successful implementation of PCI for AMI patients PCI group,80 cases of AMI patients using medicine conservative treatment for drug conservative treatment group）and 80 cases of healthy check-up AT-Ⅲ,D-dimer level.Results AT-Ⅲ,D-dimer level of PCI group and conservative treatment group before treatment compared with healthy controls had significant difference （P〈0.05）;AT-Ⅲ,D-dime level of PCI group and conservative treatment group after treatment of 3 d,7 d,14 d compared with 12 h after treatment had significant difference （P〈0.05）;D-dimer levels of PCI group 12 h after treatment compared with before treatment had significant difference（P〈0.05）.Conclusion Through continu-ous monitoring AMI treatment during the AT-Ⅲ and D-dimer level,to guide the treatment and prognosis of AMI and to reduce the risk of perioperative complications after PCI treatment.
