海南省三甲医院剖宫产的非医学因素分析

摘　　要：目的对海南省三甲医院2011年产妇剖宫产情况进行分析,了解剖宫产的非医学因素,为降低剖宫产率提供科学的依据。方法对海南省两家三甲医院2011年住院足月分娩病例进行回顾性分析。结果 1住院分娩产妇781例,剖宫产例数580人,剖宫产率为74.26%。2高龄产妇组与非高龄产妇组间差别有统计学意义(χ2=8.589,P=0.003);首次分娩组与非首次分娩组间差别有统计学意义(χ2=11.691,P<0.001)。3 Logistic回归分析结果显示高龄产妇(OR=2.059)、首次分娩(OR=4.167)、新生儿为男孩(OR=1.577)的剖宫产率均较高,是剖宫产率增加的主要因素。结论影响剖宫产的非医学因素为高龄产妇、首次分娩和新生儿性别,加强孕妇产前心理辅导、相关生产知识教育和严格把握剖宫产指征,以减少非医学指征的剖宫产率。Objective To analyze the non-medical factors of cesarean section in third-level grade-A hospitals of Hainan during 2011,so as to provide scientific basis for reducing cesarean section rate. Methods The clinical data of cases of hospitalized full-term delivery in two third-level grade-A hospitals of Hainan during 2011 were analyzed retrospectively. Results ①Among 781 cases of hospitalized full-term delivery, there were 580 cases of cesarean section, with cesarean section rate of 74.26%. ② There were statistically significant differences between advanced maternal age group and non-advanced maternal age group（P = 0.003）, first delivery group and non-first delivery group（P〈0.001）. ③Logistic regression analysis showed that advanced maternal age（OR = 2.059）, first delivery（OR = 4.167） and male neonate（OR= 1.577） were the main factors of increased cesarean section rate. Conclusions The non-medical factors of cesarean section are advanced maternal age, first delivery and neonate sex. Strengthened maternity psychological counseling before delivery, intensive education of related knowledge of production and strict grasp of indications of cesarean section can reduce the cesarean section caused by non-medical factors.
