苏芬太尼复合不同剂量地佐辛用于老年患者术后镇痛的临床研究

摘　　要：目的探讨苏芬太尼复合不同剂量地佐辛用于术后老年患者自控静脉镇痛（PCIA）的效果及安全性。方法选取80例2012年1月至2014年1月在上海市第六人民医院金山分院全身麻醉下腹部手术患者，采用随机数字表法分为C、D1、D2、D3组 （每组20例）：C组术后静脉镇痛给予苏芬太尼2 μg/kg；D1组给予苏芬太尼1 μg/kg ＋ 地佐辛0.1 mg/kg；D2组给予苏芬太尼1 μg/kg ＋ 地佐辛0.2 mg/kg；D3组给予苏芬太尼1 μg/kg＋ 地佐辛0.3 mg/kg。每组均给予甲氧氯普胺10 mg。观察并记录患者镇痛后3、6、24、48 h疼痛视觉模拟量表（VAS） 评分、镇静评分，记录48 h内患者自控静脉镇痛（PCIA）按压次数，术中苏芬太尼消耗剂量及不良反应发生情况。结果4组患者术后各时间点VAS评分、镇静评分差异无统计学意义（P﹥0.05）；48 h内PCIA按压次数及术中苏芬太尼给药剂量差异无统计学意义（P﹥0.05）；D3组术后24 h头晕、恶心、呕吐、尿潴留发生率［分别为25.0%（5/20）、20.0%（4/20）、20.0%（4/20）、10.0%（2/20）］高于D1组［分别为10.0%（2/20）、5.0%（1/20）、10.0%（2/20）、0.0%（0）］和D2组［分别为15.0%（3/20）、10.0%（2/20）、10.0%（2/20）、0.0%（0），P﹤0.05］。结论苏芬太尼1 μg/kg复合地佐辛0.1 mg/kg适用于老年患者术后自控静脉镇痛。ObjectiveTo study the effect of sufentanyl combined with different doses of dezocine on postoperative analgesia in elderly patients. MethodsA total of eighty patients from Januclry 2012 to Janucary 2013 at Jinshan SubHospital of No.6 Affiliated Hospital of Shanghai Jiaotong University scheduled for elective abdomen surgery were randomly assigned to four groups： control group （C） （2 μg/kg sufentanyl plus 10 mg metoclopramide）,dezocine group 1 （D1）（1 μg/kg sufentanyl plus 0.1 mg/kg dezocine plus 10 mg metoclopramide）,dozecine group 2 （D2）（1 μg/kg sufentanyl plus 0.2 mg/kg dezocine plus 10 mg metoclopramide ）and dozecine group 3 （D3）（1 μg/kg sufentanyl plus 0.3 mg/kg dezocine plus 10 mg metoclopramide） with 20 cases in each group. The indexes of the Visual analogue scale （VAS） and sedation score were recorded at 3 h,6 h,24 h,48 h after operation. Results The VAS and Sedation score at 3 h,6 h,24 h,48 h after operation had no significant difference among the four groups（P﹥0.05）. The pressing times of PCIA during 48 h and the consumed dosage of sufentanyl during operation had no significant difference among the four groups（P﹥0.05）.The side effects of dizziness, nausea, vomiting, retention of urine, in group D3[25.0% （5/20）,20.0%（4/20）,20.0%（4/20）,10.0%（2/20）] was higher than that in group D1 [10.0%（2/20）,5.0%（1/20）,10.0%（2/20）,0.0%（0）] or group D2 [15.0%（3/20）,10.0%（2/20）,10.0%（2/20）,0.0%（0）],respectively （P﹤0.05）. Conclusion Sufentanyl （1 μg/kg） combined with dezocine （0.1 mg/kg） is suitable for postoperative PCIA in elderly patients.
