摘 要:目的观察鱼油脂肪乳剂预处理对大鼠肺缺血-再灌注损伤的保护作用及可能机制。方法 32只SD大鼠随机分为四组:预给鱼油脂肪乳剂再灌注组(FOFE组)、预给鱼油脂肪乳剂加锌原卟啉再灌注组(FOFE-Z组)、预给生理盐水再灌注组(Saline组)和假手术组(Sham组)。监测并采集大鼠缺血末和实验末时的HR、BP并计算RPP,检测平衡末、缺血末、实验末经股动脉处采血的动脉血气;测定左肺组织湿干重比(W/D)和肺泡损伤评分IQA;检测左肺组织中IL-6和MPO含量以及NF-κB和HO-1(血红素氧化酶-1heme oxygenase-1)的表达,观察左肺组织形态结构改变。结果与FOFE组比较,缺血末和实验末FOFE-Z组的HR明显减慢,SBP和RPP明显降低,实验末Sham组SBP和RPP明显升高(P<0.05);FOFE-Z组和Saline组PaO2明显降低,IL-6含量、IQA评分和W/D明显升高(P<0.05或P<0.01),FOFE-Z组的BE明显降低(P<0.05);Saline组MPO含量明显升高(P<0.05);FOFE-Z组的NF-κB mRNA表达明显升高,FOFE-Z组和Sham组的HO-1的表达明显降低(P<0.05)。与FOFE-Z组比较,缺血末和实验末Sham组HR明显增快,SBP和RPP明显升高(P<0.05),实验末Saline组HR明显增快(P<0.05)。与Sham组比较,实验末FOFE组、FOFE-Z组和Saline组的pH明显降低,IL-6含量和IQA评分明显升高(P<0.05或P<0.01),FOFE-Z组和Saline组的PaO2明显降低,W/D明显升高(P<0.01),FOFE-Z组的BE明显降低(P<0.05),Saline组MPO含量和NF-κB的mRNA表达明显升高(P<0.05)。结论鱼油脂肪乳剂预处理可减轻大鼠肺缺血-再灌注损伤,其机制可能与增强HO-1的活性和表达有关。Objective To investigate the effects ofω-3fish oil fat emulsion on lung ischemiareperfusion(IR)injury in rats.Methods Thirty-two Sprague Dawley rats were randomly divided into 4groups(n=8each):IR+ω-3fish oil fat emulsion(group FOFE),receiving 10% fish oil Omegaven(5mg/kg)30min before IR;IR+heme oxygenase-1inhibitor+ω-3fish oil fat emulsion group(group FOFE-Z),IR+normal saline group(group Saline)and Sham operation group(group Sham).Hemodynamic data were monitored regularly and arterial blood gas analysis was performed before the clamping,at the end of ischemia and at the end of experiment.Wet lung weight to dry lung weight ratio(W/D),the levels of IL-6,myeloperoxidase(MPO)and the expression of nuclear factor-κB(NF-κB)and heme oxygenase-1(HO-1)mRNA of lung tissue were determined.The morphological and ultrastructural changes of lung tissue were observed under light microscope and electron microscope and lung injury score was calculated as well.Results Compared with group FOFE,HR,SBP and RPP in group FOFE-Z were decreased in the end of ischemia and whole experiment,SBP and RPP in group Sham were increased in the end of experiment(P〈0.05);PaO2 were lower and IL-6,IQA,W/D were higher in groups FOFE-Z and Saline(P〈0.05 or P〈0.01);BE in group FOFE-Z decreased and MPO in group Saline increased(P〈0.05);NF-κB mRNA in group FOFE-Z were more and HO-1in groups FOFE-Z and Sham were less(P〈0.05).Compared with group FOFE-Z,HR,SBP and RPP in group Sham were increased in the end of ischemia and whole experiment,HR in group Saline was increased in the end of ischemia(P〈0.05).Compared with group Sham,pH was decreased and IL-6,IQA were increased in the other three groups(P〈0.05 or P〈0.01),PaO2 was lower and W/D was higher in groups FOFE-Z and Saline(P〈0.01),BE was decreased in group FOFE-Z MPO and NF-κB mRNA were increased in group Saline(P〈0.05).Conclusion Pretreatment with fish oil fat emulsion could attenuate lung ischem
