摘  要:目的观察盐酸戊乙奎醚对大鼠失血性休克致急性肺损伤肺组织CD40和CD40L表达的影响,探讨盐酸戊乙奎醚的肺保护作用机制及对预后的影响。方法选取健康成年大鼠40只,随机均分为对照组(S组),失血性休克模型组(A组),P1组、P2组、P3组分别于放血前即刻、放血完成后即刻、再灌注前即刻给予盐酸戊乙奎醚2mg/kg。除S组外其他组动物制作失血性休克再灌注急性肺损伤模型。采用免疫组织化学检测肺组织CD40、CD40L的表达,采用ELISA法检测血浆TNF-α、IL-10含量,用HE染色观察肺组织的病理改变。结果 A组肺组织明显增大,肺叶表面有散在的大小不等的红色斑点,切面可见粉红色液体渗出,镜下见肺泡壁增厚,肺间质水肿,肺泡腔内有大量的红细胞渗出及中性粒细胞浸润,病理形态证实急性肺损伤模型建立成功。P1组、P2组和P3组肺组织的炎症病理改变较A组减轻,炎性细胞浸润减少,S组肺组织形态正常。A组、P1组、P2组和P3组W/D明显大于、CD40和CD40L表达明显高于S组(P<0.05)。而P1组、P2组和P3组W/D明显小于、CD40和CD40L表达明显低于A组(P<0.05)。P1组、P2组W/D明显小于、CD40和CD40L表达明显低于P3组(P<0.05),P1组、P2组差异无统计学意义。A组、P1组、P2组和P3组TNF-α、IL-10含量明显高于S组(P<0.05),P1组、P2组和P3组TNF-α、IL-10含量明显低于A组(P<0.05),P1和P2组TNF-α、IL-10含量明显低于P3组(P<0.05),P1组和P2组差异无统计学意义。结论盐酸戊乙奎醚抑制大鼠失血性休克复苏肺组织CD40、CD40L的表达,进一步降低TNF-α、IL-10的合成和表达,从而减轻肺损伤,具有一定的肺保护作用。并且,早期给予盐酸戊乙奎醚对预后改善明显。Objective To investigate the effects of penehyclidine hydrochloride(PHCD)on expression of CD40,CD40 Lin rats with hemorrhagic shock induced Acute Lung Injury and the effects of administration at different time points on prognosis.Methods Forty normal Wistar rats weighing200-250 g were randomly divided into 5groups:control group(group S,n=8),hemorrhagic shock group(group A,n=8)and PHCD treatment group 1,2,3(group P1,P2,P3,n=8each).PHCD treatment group was given Penehyclidine 2mg/kg.Except the rats in control group,rest of the rats were subjected to hemorrhagic shock models with lung injury.Expression of CD40 and CD40Lin lung tissue were measured by immunohistochemistry.The plasma concentration of TNF-αand IL-10 were measured by ELISA.Pathologic change in lung tissue were observed by light microscope.Results In group A,the lung tissue increased obviously,the lung surface scattered with red spots,pink fluid exudation from section.We observed alveolar wall thickening,interstitial pulmonary edema,exudation of RBC and neutrophil infiltration in the alveolar cavities by microscope.Pathological morphology confirmed acute lung injury model was established successfully.Inflammatory pathological change of lung tissue and infiltration of inflammatory cells in lung tissue in group P1,P2,P3 was reduced compare to group A.Pulmonary morphology in group S was normal.Compared with group S,W/D and expression of CD40 and CD40L were increased significantly in group A,P1,P2,P3(P〈0.05).While W/D and expression of CD40 and CD40Lwere decreased significantly in group P1,P2,P3 than those in group A(P〈0.05).W/D and expression of CD40 and CD40Lwere decreased significantly in group P1,P2 than those in group P3(P〈0.05).There were no significant differences in group P1,P2.Plasma concentration of TNF-α,IL-10 were significantly higher in group A,P1,P2,P3 than those in group S(P〈0.05).TNF-α,IL-10 were significantly lower in group P1,P2,P3 than those in group A(P〈0.05).TNF-α,IL-10 were significant
