挤压综合征性肾损伤时HIF-1α变化及与肾血流量和血液流变学的关系的研究  被引量:2

摘  要:目的通过CoCl2上调HIF-1α的表达和棘霉素抑制HIF-1α转录活性,观察挤压综合征性肾损伤时HIF-1α表达与肾血流量及血液流变学的关系。方法 SD大鼠,随机分为假手术组、模型组、CoCl2组、棘霉素组。检测肾血流量、血液流变学指标、肾功能和形态学改变以及肾组织HIF-1α表达,Image Pro plus6.0图像分析软件进行半定量分析。结果模型组肾血流量明显减少,血液流变性指标和Cr、BUN增高(P<0.05),肾小管上皮肿胀,部分管腔闭塞,坏死,管型可见,肾HIF-1α少量表达。与模型组比较,CoCl2组HIF-1α表达增加(P<0.05),其余指标异常有所改善(P<0.05);棘霉素处理HIF-1α表达未增加,其余指标异常进一步加重(P<0.05)。结论 HIF-1α表达增加可改善血液流变性,增加肾血流量,对挤压综合征性肾损伤具有一定保护作用。Objective To observe hemorheology and renal blood flow in acute kidney injury after crush syndrome after using up-regulation of hypoxia-inducible factor-1α(HIF-1α) and inhibitory activity with echinomycin. Methods 32 male SD rats were randomly divided into sham group, model group, CoCl2(preconditioning) group and echinomycin(preconditioning) group. The indexes, including hemorheology, renal blood flow(RBF), blood urea nitrogen(BUN), serum creatinine(Cr), histopathology and immunohistochemistry staining for HIF-1α, were detected. The average optical density(OD) values of HIF-1α were analyzed by Imagepro-plus 6.0. Results Immunohistochemistry suggested that a small amount of HIF-1α protein expressed in renal tubular epithelial cell in model group. The renal blood flow decreased significantly. Indexes of hemorheology, Cr and BUN obviously increased. Histopathological examination suggested the swelling of renal tubular epithelial cell and the cast formation obviously(tubular epithelial swelling, partial luminal occlusion, necrosis, casts visible). In the CoCl2 group, expression of HIF-1α markedly increased, with the rest abnormal indexes improving. In the echinomycin group, inhibitory activity of HIF-1α deteriorated, with the rest abnormal indexes further decreasing. Conclusion The expression of HIF-1α may improve hemorheology and increase renal blood flow so as to improve renal function and reduce tubular injury.
