摘 要:目的:通过对比腰椎骨髓信号在高、低场磁共振各序列上的差异,找出高、低场磁共振对病灶检查的局限性,避免不同场强磁共振的不足,以积累诊断经验。方法收集2012年9月~2012年12月腰椎退变14例,腰椎外伤8例,腰椎骨转移瘤5例,在Siemens verio 3.0T和日立Airis‐Ⅱ0.3T磁共振仪上分别做检查,两次检查在同一天完成,期间未做任何治疗,两名副主任医师单盲法阅片,分析14例腰椎退变中30处局灶性黄骨髓,8例腰椎外伤中10处骨髓水肿,5例腰椎转移瘤中16处转移灶及2例瘤周水肿的信号在高、低场磁共振各序列的表现差异。结果在Siemens verio 3.0T磁共振仪上,各序列信号清晰,但T2加权像对水肿显示不清;在Airis‐Ⅱ0.3T磁共振仪,各序列图像较模糊,尤其 T2加权像对局灶性黄骨髓显示不清,但对水肿信号的显示明显优于高场T2加权像。结论高场磁共振,各序列图像清晰,低场磁共振图像模糊;低场T2加权像对于局灶性黄骨髓的显示明显差于高场;而对于水肿的显示明显优于高场 T2加权像,高场磁共振的T2压脂序列是不可缺少的。Objective Through the comparison of the difference of lumbar bone marrow signal in high ,low field magnet‐ic resonance ,to and out the limitation of high ,low field magnetic resonance to avoid the deficiency of different magnetic resonance ,in order to improve the diagnostic level .Methods From September 2012 to December 2012 ,14 cases of degen‐erative lumbar spine ,8 cases of lumbar injury ,5 cases of lumbar spine metastasis were checked in the Siemens Verio 3 .0 T and Hitachi Airis‐Ⅱ 0 .3 T magnetic resonance instrument respectively ,two examination were perforned in the same day without any treatment .Two deputy chief physician analysed 30 focal yellow bone in 14 cases of degenerative lumbar spine , 10 bone marrow edema in 8 cases of lumbar trauma ,16 lesions in 5 cases of lumbar vertebral metastases and 2 cases of per‐itumorous edema signal differences in high & low field magnetic resonance by single blind method respectively .Results In the Siemens Verio 3.0T magnetic resonance instrument ,the signal is clear ,but the T2 weighted image display of edema is not clear ;in Airis‐Ⅱ 0 .3T magnetic resonance instrument ,the image is not clear ,especially ,the focal yellow marrow in T2 weighted image is not clear ,but the edema signal display is superior to high field T2 weighted image .Conclusion The image of high field magnetic resonance is clear ,low field magnetic resonance image is not ;low field T2 weighted image for display focal yellow bone marrow is significantly worse than the high field and better than high field T 2 weighted image for edema .High field magnetic resonance T2 fat saturation sequence is indispensable .
