下睑松垂患者下睑袋整形术改良体会

摘　　要：目的：探讨下睑松垂患者下睑袋整形手术的改良。方法：对121例（242眼）下睑袋患者进行整形手术，其中男性30例，女性91例，平均年龄51.2岁。针对下睑松垂患者下睑袋的病理特点，取下睑缘切口，垂直切开皮肤，暴露眼轮匝肌，向眶隔以远稍分离1～2mm，以45＆#176;角向下斜行将眼轮匝肌剖开，显露眶隔，去除疝出多余脂肪并重置，眶隔加固后，缝合皮肤切口。患者均于术后7天拆除切口缝线。结果：术后121例患者的下睑袋和下睑松垂均得到了满意的修复，随访3～24个月，眶下区均得到年轻化改善，下睑及睑缘形态自然，无下睑外翻和睑球分离。结论：改良的下睑袋整形技术，对下睑松垂患者的病理特点有比较强的针对性，是一种比较适合下睑松垂患者的下睑袋整形方法。Objective To investigate the modification of lower eyelid blepharoplasty on patients with palpebral relaxation. Methods 121 cases （242） of patients with palpebral bags accept low eyelid blepharoplasty in which, 30 were male,91 were female,with an average age of 51.2 years.According to the pathologic feature of palpabral bags with palpebral relaxation, the low eyelid incision is chosen, a vertical dissection to the skin with exposing the orbicularis oculi muscle, and dissection towards the orbital septum about 1- 2mm,45- degree oblique split orbicularis oculi muscle to expose the orbital septum, excess fat is removed and reset.When the orbital septum is reinforced,the skin is closed.All patients remove suture in the 7 days. Results 121 cases with palpebral bags and relaxation have been followed- up for 3- 24 months in which all achieved satisfactory results. There are improvements in suborbital area, natural shape of lower eyelid without ectropion and lidconjunctival contact failure. Conclusion The modified lower eyelid blepharoplasty aiming at the pathological features of patients with palpebral relaxation,is a suitable method for patients with lower eyelid palpebral relaxation.
