摘　　要：目的探讨螺旋CT增强扫描对上腹部实质性脏器闭合性外伤的诊断、分期价值。方法回顾性分析58例上腹部实质性脏器闭合性外伤患者的CT表现,并进行CT分期,与临床治疗结果(手术或非手术治疗)对照分析。结果 CT表现为脾损伤33例,其中Ⅰ级3例,Ⅱ级4例,Ⅲ级19例,Ⅳ级7例;肝损伤10例,其中Ⅰ级3例,Ⅱ级2例,Ⅲ级3例,Ⅳ级2例;肾损伤14例,其中Ⅰ级3例,Ⅱ级5例,Ⅲ级4例,Ⅳ级2例;胰腺损伤4例,其中Ⅰ级1例,Ⅱ级1例,Ⅲ级2例,Ⅳ级0例。结论增强螺旋CT对上腹部闭合性实质性脏器外伤诊断、分级有重要作用。Objective To evaluate the diagnostic and staging value of spiral CT（SCT）in blunt solid upper abdominal injury（BSUAI）. Methods CT findings and grading systems in 58 cases with BSUAI were analyzed retrospectively and compared with clinic treatment outcomes. Results All cases of BSUAI were detected by SCT.Among spleen injury（ n=33）,gradeⅠ,gradeⅡ,gradeⅢ,and gradeⅣhave 3,4,19,and 7cases respectively.Among liver injury（n=10）,gradeⅠ,grade Ⅱ,grade Ⅲ,and grade Ⅳ have 3,2,3,and 2cases respectively.Among kidney injury（n=14）,grade Ⅰ,gradeⅡ,gradeⅢ,and gradeⅣhave 3,5,4,and 2 cases respectively.Among pancreas injury（n=14）,gradeⅠ,gradeⅡ,gradeⅢ ,and grade Ⅳ have 1,1,2,and 0 cases respectively. Conclusion SCT enhanced scan is of important value in diagnosing and staging of BSUAI.
