摘 要:目的探讨颅骨标本中鼻内镜视野下上颌窦裂孔区钩突附着位置。方法收集骨性鼻腔外侧壁保存完好的成人颅骨69具,鼻内镜下确定上颌窦裂孔大体标志和主要结构,观察钩突上缘与中鼻甲、颅底、眶内侧壁的附着关系并记录。结果上颌窦裂孔区钩突头端附着情况分为7种方式:1纸样板型;2中鼻甲型;3前颅底型;4纸样板-中鼻甲型;5中鼻甲-颅底型;6颅底-纸样板型;7颅底-纸样板-中鼻甲型。结论上颌窦裂孔的内镜解剖是进行鼻内镜下鼻窦手术及鼻眼相关等手术的解剖基础,术前熟悉上颌窦裂孔区钩突附着位置的应用解剖是提高手术安全性和疗效的重要前提。Objective To investigate the attachment location of unicate process in maxillary hiatus in skull under nasal endoscope. Methods 69 adult skull samples with intact lateral wall of bony nasal cavity were collected. General anatomical landmarks and structure of maxillary hiatus were determined under nasal endoscope. The attachment of uncinate process was observed. The relation between superior margin of uncinate process and the corresponding middle turbinate, skull base, ethmoid bulla, and middle orbit wall were recorded. Results Among all the 69 skulls, 7 attachment patterns were confirmed, including lamina papyracea type, middle turbinate type, anterior cranial fossa type, lamina papyracea-middle turbinate type, middle turbinate-anterior cranial fossa type, lamina papyracea-anterior cranial fossa type and lamina papyracea-anterior cranial fossa middle turbinate type. Conclusion The endoscopic anatomy of maxillary hiatus is the basis of both nasal endoscopic surgery and orbital-related endoscopic surgery. Acquaintance with the applied anatomy of unicate process attachment is an important prerequisite for reliability and effect of these surgical procedures.
