EV71型重症手足口病患儿心肌酶、肌钙蛋白Ⅰ、肺炎支原体抗体检测的临床意义

摘　　要：目的：探讨心肌酶、肌钙蛋白Ⅰ（ cTnⅠ）、肺炎支原体抗体检测在EV71型重症手足口病中的临床意义。方法选取2012年5月～2014年5月收治的215例EV71型重症手足口病患儿为观察组，同时选取普通EV71型手足口病病例120例作为对照组，分别检测两组血清心肌酶、cTnⅠ的水平，肺炎支原体IgM抗体，并分析观察组患儿治疗前后心肌酶、cTnⅠ的变化情况。结果观察组心肌酶、cTnⅠ高于对照组，差异具有统计学意义；肺炎支原体IgM抗体阳性率高于对照组；观察组、对照组患儿治疗后心肌酶、cTnⅠ水平较治疗前明显下降，差异具有统计学意义。结论手足口病病程中心肌较易受到侵犯，重症病例心肌酶、cTnⅠ升高更为明显；观察组和对照组相比较，肺炎支原体抗体的阳性率显著高于对照组，肺炎支原体感染可能为导致手足口病加重的一个诱因，手足口病心肌酶、cTnⅠ、肺炎支原体抗体检测的临床意义重大，可作为重症手足口病患儿临床检测的常规指标。Objective To investigate the clinical significance of testing myocardial enzyme,troponinⅠand antibody of mycoplas-monia（ MP-IgM） in children with severe hand-foot-mouth disease.Methods 215 cases of EV71 type with severe hand-foot-mouth disease from may 2012 to may 2014 in the Weifang people＇s hospital were selected.As the observation group and 120 cases of regular hand-foot-mouth disease were chosen as the control group;the level of serum myocardial enzyme,cTnⅠ,anti-mycoplasmonia in the two groups were tested,the indications in the observation group before and after the treatment were analyzed.Results The level of myocardial enzyme,troponinⅠand the positive rate of anti-MP-IgM in the observation group were higher than those in the control group and the differences were statistical significant （ P〈0.01）;the level of myocardial enzyme,troponin Ⅰin observation group after the treatment were lower than those before the treatment, the differences were statistical significant（P〈0.01）.Conclusion It is common for hand-foot-mouth disease to invade the myocardium and the level of myocardial enzyme and troponinⅠwere higher in severer cases.Compared with the control group,the positive rate of anti-MP-IgM was higher and the infection of mycoplasmonia may be one cause that aggravates hand-foot-mouth disease.Besides,testing myocardial enzyme, cTnⅠ,MP-IgM is of great clinical significance with hand-foot-mouth disease,especially for those severe cases,it is right to consider the index as the conventional examination.
