热放疗、放化疗和单纯放疗治疗食管癌的临床疗效比照分析

摘　　要：目的探讨热放疗、放化疗和单纯放疗治疗食管癌的临床效果,为日后选择治疗方案提供指导。方法回顾分析自2012年1月—2014年6月期间该院收治的晚期食管癌患者75例,75例患者随机分为热放疗组(n=25)、放化疗组(n=25)和纯放疗组(n=25)。3组患者均接受常规性放射治疗,放化疗组患者在放射治疗基础上采用化疗治疗,热放疗组患者在放射治疗的基础上采用热疗治疗。比较3组患者治疗4周后的临床效果和不良反应。结果经治疗结束4周后,热放疗组的治疗总有效率为68.00%,放化疗组的治疗总有效率为84.00%,纯放疗组的治疗总有效率为56.00%。放化疗组和纯放疗组的治疗效果,差异有统计学意义(P<0.05)。放化疗组患者的不良反应发生率高于纯放疗组和热放疗组,差异有统计学意义(P<0.05)。结论放疗结合化疗治疗食管癌的临床效果佳,但不良反应发生率高,应根据患者的病情及需要采用合适的治疗方法。Objective To observe the clinical effects between thermal radiation, radiochemotherapy and radiation therapy on esophageal cancer so as to provide guidance for choosing the treatment in the future. Methods 75 cases with advanced esophageal cancer admitted in our hospital from January 2012 to June 2014 were analyzed retrospectively. And the 75 patients were randomly divided into the thermal radiation group（n=25）, radiochemotherapy group（n=25） and the radiation therapy group（n=25）. All the pa- tients were given the routine radiotherapy, in addition, the radiochemotherapy group was given chemotherapy, and the thermal ra- diation group was given heat treatment. And the clinical effects and adverse reactions were compared between the 3 groups after 4 weeks of treatment. Results 4 weeks after treatment, the total effective rate of the thermal radiation group was 68.00%, and that of the radiochemotherapy group was 84.00%, and that of the radiation therapy group was 56.00%. The difference in treatment effect between the radiation therapy group and the radiochemotherapy＇group was statistically significant （P〈0.05）. And the incidence of adverse reactions of the radiochemotherapy group was higher than that of the radiation therapy group and thermal radiation group （P〈0.05）. Conclusion The clinical effect of radiotherapy combined with chemotherapy in the treatment of esophageal cancer is better but the incidence of adverse reactions is higher. Appropriate treatment should be adopted according to the condition and need of the patient.
