COOK宫颈扩张球囊联合催产素引产的效果及安全性分析

摘　　要：目的比较分析COOK宫颈扩张球囊联合催产素引产及单纯使用催产素引产的临床效果及安全性。方法将2013年2月—2014年1月在该院引产分娩的单胎足月的初产妇共123例,分为观察组(球囊联合催产素)及对照组(单用催产素),比较两组剖宫产率、第一产程时间、产后出血、胎儿窘迫、羊水污染、产后盆腔感染等情况。结果两组比较,观察组剖宫产率较低为21.05%,对照组为43.94%,P<0.05,观察组第一产程时间较短,时间为(4.23±2.28)h,小于对照组(5.38±1.97)h,P<0.05,差异有统计学意义;产后出血率观察组(2.2%)较对照组(16.2%)稍低,羊水污染率观察组(31.1%)较对照组(10.8%)稍多,P<0.05,差异有统计学意义,而胎儿窘迫及产后盆腔感染两者差异无统计学意义。结论 COOK宫颈扩张球囊联合催产素引产具有较好的临床效果及安全性,值得在临床推广应用。Objective To compare and analyze the clinical efficacy and safety between COOK cervical dilation balloon combined with oxytocin and oxytocin alone for labor induction. Methods A total of 123 primiparas with full-term single birth underwent labor induction and delivery in our hospital from February 2013 to January 2014 were divided into the observation group（COOK cervical dilation balloon combined with oxytocin） and the control group（single oxytocin）. The rate of cesarean section, duration of the first stage of labor, postpartum hemorrhage, fetal distress, amniotic fluid contamination and postpartum pelvic infection were compared between the two groups. Results The rate of cesarean section of the observation group was 21.05%, lower than 43.94% of the control group, P〈0.05; the duration of the first stage of labor of the observation group was 4.23 ±2.28 h, shorter than 5.38±1.97 h of the control group, the difference was statistically significant, P〈0.05; the rate of postpartum hemorrhage of the observation group（2.2%）was lower than that of the control group（16.2%）, the rate of amniotic fluid contamination of the observation group（31.1%） was more than that of the control group（10.8%）, the differences were statistically significant, P〈0.05, while the differences in fetal distress and postpartum pelvic infection between the two groups were not significant. Conclusion COOK cervical dilation balloon combined with oxytocin for labor induction has a good clinical outcome and safety, which is worthy of clinical application and promotion.
