摘 要:目的探讨尿酸(UA)与同型半胱氨酸(Hcy)、超敏C反应蛋白(hs-CRP)及尿白蛋白排泄率(UAER)的关系,及其对2型糖尿病早期肾病(DN)的影响。方法在三水地区进行痛风及高尿酸血症流行病学调查样本中随机抽查87例2型糖尿病早期肾病患者的UA、Hcy、hs-CRP和UAER水平,同时根据尿酸水平分为高尿酸组和尿酸正常组,并随机抽取30例2型糖尿病不伴早期肾病患者为对照组,比较三组的UA、Hcy、hs-CRP和UAER水平,分析尿酸与2型糖尿病早期肾病的相关性。结果 2型糖尿病早期肾病患者UA、Hcy和hs-CRP水平明显高于不伴早期肾病患者(P<0.01),并且2型糖尿病早期肾病伴高尿酸患者Hcy和UAER水平明显高于尿酸正常组(P<0.01),血尿酸水平与UAER、Hcy呈正相关(P<0.05)。结论尿酸水平的高低与2型糖尿病早期肾病密切相关,高尿酸血症可能加速2型糖尿病早期肾病的进程,对2型糖尿病早期肾病预后有警示意义。Objective To study uric acid(UA) and homocysteine(Hcy),hypersensitive C-reactive protein(hs-CRP) and the relationship between urinary albumin excretion rate(UAER),and the early nephropathy(DN) in type 2 diabetes.Methods From the epidemiology investigation samples of gout and high uric acid hematic disease in Sanshui district,87 cases of patients with early nephropathy in type 2 diabetes were randomly selected. Their uric acid, homocysteine,hypersensitive c-reactive protein, and urinary albumin excretion rate were determined. According to the uric acid level,the patients were divided into high uric acid group and uric acid normal group. 30 patients with type 2 diabetes is not associated with early kidney disease patients were chosen as control group. Uric acid, homocysteine, hypersensitive creactive protein, urine albumin excretion rate or the three groups were analyzed for the correlation between uric acid and type 2 diabetes with early nephropathy. Results In early nephropathy in type 2 diabetes cases, the uric acid,homocysteine, hypersensitive c-reactive protein levels were significantly higher than those without early nephropathy patients(P〈0.01). For the type 2 diabetes mellitus patients with high uric acid with early nephropathy, the homocysteine levels and urinary albumin excretion rate were significantly higher than those of normal group(P〈0.01). Serum uric acid level and urine albumin excretion rate, and homocysteine were positively correlated(P 〈0.05). Conclusion Uric acid level is closely associated with early nephropathy in type 2 diabetes, high serum uric acid level may accelerate the process of early nephropathy in type 2 diabetes. It could be a significant marker for prognosis of early nephropathy in type 2 diabetes.
