摘  要:目的探讨言语诱发听性脑干反应(speech evoked auditory brainstem response,s-ABR)的影响因素,揭示s-ABR更多的电生理特性。方法将24名健康成人(24耳)的s-ABR采用两种刺激率(11.1次/s、20.0次/s)和两种刺激强度(80d B SPL、60d B SPL)进行2×2×6析因实验,分析s-ABR波形评分值、各主波潜伏期及幅值。结果 s-ABR波形评分值差异无统计意义(p>0.05);各主波潜伏期的刺激率差异无统计学意义;各幅值(除外Ⅴ波)的刺激率差异亦无统计学意义;大部分主波潜伏期(除外A波、F波及O波)的强度差异有统计学意义(p<0.05),大部分主波幅值(除外Ⅴ波和O波)的强度差异则无统计学意义;波形量化评分、各主波潜伏期和幅值均无刺激率和刺激强度的交互效应(p>0.05)。结论 s-ABR测试的刺激率和刺激强度在一定范围内没有相互影响,测试中可根据需要选择刺激率和刺激强度。Objective To study variables affecting speech evoked auditory brainstem response (s-ABR) and report its electrophysiological characteristics. Methods Effects of stimulus rates (11.1/s and 20.0/s) and stimulus intensities (80 dB SPL and 60 dB SPL) on s-ABRs were studied in 24 healthy adults (24 ears), using a 2 × 2 × 6 factorial experimental design. s-ABR waveform scores, latencies and amplitudes of the feature peaks were analyzed. Results Stimulus rate had no effects on waveform scores of all s-ABRs and latencies of feature peaks (p〉0.05), or amplitudes (except for peak V). Stimulus inten?sity seemed to affect latencies of feature peaks (except for peak A, F and O) (p〈0.05), but not amplitudes of feature peaks (ex?cept for peak V and O). Different stimulus rate and intensity combinations had little effects on s-ABR waveform scores, laten?cies or amplitudes of feature peaks. Conclusion The characteristics of s-ABR are maintained across a range of stimulus rates and intensities, and not affected by different combinations of stimulus rate and intensity. A relatively wide range of stimulus intensities and rates can be selected based on needs in s-ABR testing.
