常规避免吸入性变应原在治疗变应性鼻炎中的作用

摘　　要：目的探讨日常避免变应原的措施对于治疗变应性鼻炎(AR)疗效的影响。方法 80例对吸入性变应原过敏的AR患者随机分为4组:A组,无治疗+不避免变应原;B组,避免变应原;C组,药物治疗;D组,药物治疗+避免变应原。对各组在第0、1、4周时进行症状评分(combined symptom score,CSS)及视觉模拟量表(visual analogue scale,VAS)评分评估。结果 1第1周末时,A、B两组患者在CSS及VAS改善程度上均没有显著性差异。C、D两组患者疗效明显优于A、B组(CSS:C组与A组:P=0.02,C组与B组:P=0.03,D组与A组:P=0.00,D组与B组:P=0.00;VAS:P均=0.00)。而D组患者优于C组(CSS:P=0.04;VAS:P=0.00)。2第4周末时,A组与B组及C组与D组在CSS及VAS方面均无显著性差异。结论避免吸入性变应原能有效提高抗过敏药物对AR的疗效,避免变应原不能作为AR治疗中的单一的治疗措施。OBJECTIVE To investigate the effect of inhaled allergen avoidance on the treatment of allergic rhinitis. METHODS Eighty patients allergic to inhaled allergen were divided into 4 groups randomly and received one of the four measures： no pharmacotherapy and no allergen avoidance（group A）, allergen avoidance only（group B）, pharmacotherapy only（group C）, pharmacotherapy and allergen avoidance（group D）. Combined symptom score（CSS） and visual analogue scale（VAS） were used to assess the efficacy of treatment before and 1 or 4 weeks after treatment onset. RESULTS 1. Both CSS and VAS showed there was no significant difference between group A and group B at the first weekend after treatment. The efficacy of group C and D was significantly superior to the other two groups at the first weekend（CSS： group C and A： P=0.02, group C and B： P=0.03, group D and A： P =0.00, group D and B： P =0.00; VAS： all P =0.00）. The improvement of efficacy on group D was significantly better than that on group C（CSS： P=0.04; VAS： P=0.00）. 2. At the fourth weekend after treatment, the significant improvement differences between both group C and D and group A and B were reduced to inexistent. CONCLUSION The avoidance of inhaled allergen can improve the effect of pharmacotherapy on allergic rhinitis patients. Allergen avoidance shouldn’t be used as a single measure to allergic rhinitis patients.
