糖调节异常患者听觉功能的临床观察

摘　　要：目的通过检测IGR患者的听力情况,了解IGR患者中听力异常情况。方法 75g OGTT筛查入选IGR(IGR组)患者30例,同时选取年龄、性别匹配正常者25名作为对照(NGT组)组。对研究对象行纯音测听及声导抗检查,并将IGR组分为两亚组,即有耳聋主诉亚组和无耳聋主诉亚组。结果 IGR组中14例(46.67%)存在听力损失,NGT组中3名(12%)存在听力损失(χ2=7.674,P=0.006)。有耳聋主诉亚组5例(80%)存在听力损失,无耳聋主诉亚组25例(40%)存在听力损失;无耳聋主诉亚组高频听阈与NGT组比较,差异有统计学意义[(29.70±21.93)vs(15.00±7.12)dB](P=0.015)。无耳聋主诉亚组与有耳聋主诉亚组伴耳鸣主诉的听力损失比较,差异无统计学意义(4例vs 6例,P=0.404)。结论 IGR患者听力损失发生率高,且听力损失可发生于出现耳聋症状前,多表现为高频听力受损。Objective To investigate auditoryfunctionin patients with impaired glucose regulationbypure-tone audiometry. Methods All the participants underwent oral glucose tolerance test.30 patients with impaired glucose regulation（IGR）were enrolled as testing subjects and 25age-and sexmatched normal person were selected as normal glucose regulation（NGT）.The subjects underwent puretone audiometry,acoustic immitance audiometry examination.IGR patients were divided into two groups with（IGR＋deafness）and without（IGR-deafness）primary deafness complaint. Results（1）Compared with the control group,hearing loss in IGR group was significantly increased（46.67% vs 12.00%,χ^2=7.674,P=0.006）.（2）5cases in IGR＋deafness group had severe hearing loss（80.00%）.10 cases in IGRdeafness group had light hearing loss（40.00%）.Compared with the control group,high-frequency hearing lossin IGR-deafness group was significantly increased（P=0.015）.（3）Hearing loss in IGR-deafness group but with the complaint of tinnitus was not statistically different from that with non-tinnitus complaint（P=0.404）. Conclusion The incidence of hearing loss is significantly increasedin patients with IGR.Hearing loss in IGR may occur before onset of symptoms of deafness,and high-frequency hearing loss is more frequent.
