妇科肿瘤患者焦虑影响因素分析

摘　　要：目的了解影响某院妇科肿瘤患者发生焦虑的影响因素,为进一步采取相应的预防干预手段提供依据。方法将2011年5月-2013年5月于该院住院的妇科肿瘤患者143例纳入,作为本次调查的研究对象,在患者入院后接受手术治疗前采用焦虑自评量表(Self-rating Anxiety Scale,SAS)对入组的143名患者进行评估,根据SAS标准分对患者进行分组,≥50分者判定为有焦虑症状,作为焦虑组,SAS<50分者作为对照组。比较焦虑组及对照组在年龄、文化程度、疾病类型、手术方式、经济水平等几方面的差异。结果焦虑组患者SAS评分为(61.3±7.5)分,对照组为(40.6±5.7)分,2组患者得分差异有统计学意义(t=15.95,P<0.001)。焦虑组与对照组相比,2组患者间疾病类型、手术方式、家庭人均年收入水平的差异有统计学意义(P<0.05),焦虑组恶性肿瘤者所占比例高于对照组,根治手术者所占比例较高,家庭人均年收入<1万元者所占比例较高。结论患病的严重程度、是否采取保守治疗以及患者治疗的经济负担是造成妇科肿瘤患者焦虑的影响因素,今后临床工作中,应针对以上因素对患者进行相应的心理疏导。Objective This study aimed to find the influencing factors of anxiety of the hospitalized gynecologic tumor patients, so as to provide the basis for further prevention interventions. Methods 143 patients were included in this study during May 2011 - May 2013. Before surgery, the patients were investigated by the self-assessment scale （SAS） for their status of anxiety. According to the scores of SAS they were then divided into the anxiety group （SAS 〉= 50） and the control group （SAS〈50）. The two groups were then compared for differences in the level of anxiety with age, education, disease situation, surgery method and economic conditions. Results The average SAS score is 61.3 ± 7.5 for the anxiety group and 40.6 ± 5.7 for the control group, with statistically significant difference （t=15.95, P〈0.001）. The two groups had significant differences in disease type, surgery method and family annual income level. In the anxiety group malignant tumor rate and radical surgery rate were higher, and the income per capital was lower. Conclusion The severity of the illness, conservative treatment, and the economic burden are the influencing factors of the anxiety among patients with gynecology tumor. The future clinical work should be aimed at the above factors during corresponding psychological counseling.
