摘　　要：目的比较城乡参保和参合居民住院医疗费用差异,分析住院医疗费用与住院时间的关系,为完善城乡居民医疗保障制度提供参考。方法选取某县2011年1月1日-2011年12月31日参保和参合居民住院患者,运用非参数秩和检验分析住院医疗费用,采用相关分析法分析住院医疗费用与住院时间的关系。结果参保居民平均住院费用(2 069.77元)高于参合居民平均住院费用(1 936.11元),平均住院日(10.25 d)大于参合患者平均住院日(8.13 d)(P<0.05);城乡居民住院医疗费用及个人支付费用和统筹支付费用均与住院时间显著正相关(P<0.05)。结论完善医疗保险制度及支付方式,减少住院时间,能有效减少各项住院医疗费用。Objective The study compared the difference in hospitalization cost between urban and rural medicare-insured and -participating residents and analyzed the correlation between hospitalization cost and length of hospital stay, aiming to provide a reference for optimizing the medical insurance system for urban and rural residents. Methods Medicare-insured and -participating residents of a county hospitalized between January 1st and December 31st of 2011 were selected. Hospitalization costs of the residents were analyzed by nonparametric rank sum test, and the correlation between hospitalization cost and length of hospital stay was analyzed by correlational analysis. Results The average hospitalization cost of insured residents （￥2069.77） was higher than that of participating residents （￥1936.11）, and the average length of hospital stay of insured patients （10.25 days） was longer than that of participating patients （8.13 days） （P〈0.05）. Hospitalization cost, copays, and deductibles of the residents showed a significant positive correlation with length of hospital stay （P〈0.05）. Conclusion Optimizing the medical insurance system and payment methods and reducing the length of hospital stay can effectively decrease various hospitalization costs.
