摘 要:目的观察大鼠重型颅脑创伤后对耳蜗功能的影响。方法将40只健康SD大鼠随机分成4组:对照组、创伤后24 h、48 h、72 h组。采用Marmaro的方法制作重型颅脑创伤模型,各组大鼠颅脑创伤前后行听性脑干反应(Auditory Brainstem Respons ABR)检测,并在光镜下观察伤后脑组织病理性变化以及耳蜗基底膜毛细胞损伤情况。结果重型颅脑创伤后各组大鼠脑组织出现脑实质点状出血,蛛网膜下腔或脑室出血,毛细血管周围水肿,神经元细胞肿胀变性等。创伤后各组大鼠ABR明显异常,平均阈值较对照组明显升高(P<0.05);创伤后耳蜗内积血、充血水肿,内、外毛细胞排列紊乱,胞浆萎缩、空化,细胞核萎缩或消失,尤以第三排外毛细胞损伤严重。结论重型颅脑创伤造成大鼠脑及耳蜗不同程度的病变及听功能障碍。Objective The aim of this study was to observe the effect of severe head trauma on cochlear function. Methods Forty healthy SD rats were randomly divided into four groups: control group, 24 hour post-trauma group, 48 hour post-trauma group, and 72 hour post-trauma group. Marmaro methodology was adopted to produce severe head trauma model. Each group of rats were under ABR tests before and after craniocerebral trauma, and the change of tissue pathological after head injury were observed under microscope. Cochlea hair cell injury was observed. Results After severe head trauma, parenchymal dotted hemorrhage, subarachnoid or intraventricular hemorrhage, edema around the capillaries, neurons swelling, degeneration and so on occurred in each group of rats. ABR of each group of rats after trauma was apparently abnormal, with the average threshold significantly higher than the control group (P〈0.05). After trauma hemorrhage, congestion and edema were found in the cochlea, the inner and outer hair ceils arranged in disorder. Cytoplasmic atrophy and cavitation, the nucleus atrophy disappeared. The third outer hair cell injury was the most serious. Conclusion Severe head trauma in rats resulted in different degrees of hearing dysfunction.
