摘 要:目的了解桂林市13 675例体检者高尿酸血症(HUA)的检出率,探讨HUA与体重指数(BMI)、血脂及空腹血糖(FBG)之间的相关性。方法对2011年某院体检中心有完整尿酸(UA)、BMI、FBG、血脂资料的13 675例体检者的体检资料进行整理分析,了解HUA的检出情况;应用多因素Logistic回归分析高尿酸水平与体重指数(BMI)、血脂及空腹血糖(FBG)之间的关系。结果 (1)HUA的总体检出率为18.7%,男性检出率高于女性(24.0%、11.60%,P<0.01)。HUA检出率随年龄增长而呈现递增趋势(P<0.01)。男女性人群HUA检出率最高的年龄段分别分布在30岁^(26.30%)和70岁~组(25.60%)。(2)高尿酸组的BMI、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、FBG均高于正常尿酸组(P<0.01),而高密度脂蛋白胆固醇(HDL-C)低于正常尿酸组(P<0.01)。(3)高TG、高LDL-C、高HDL-C、BMI超标、男性对HUA的风险度OR值(95%CI)分别为2.30(2.05~2.57)、1.29(1.13~1.46)、0.85(0.75~0.96)、2.52(2.27~2.79)、1.73(1.56~1.91)。结论桂林市13 675例体检者HUA的检出率较高;BMI超标、高TG、高LDL-C、男性是HUA的独立危险因素,而高HDL-C是HUA的保护因素。Objective This study aimed to determine the detection rate of Hyperuricemia (HUA) in 13675 people conducted physical examination and to explore the correlation between hyperuricemia and body mass index (BMI), blood lipid metabolism and blood glucose (FBG). Methods The uric acid (UA), BMI, FBG, blood lipid data of 13675 people who conducted physical examination in 2011 were analyzed by multiple-factor logistic regression method for the correlation of these body indexes with hyperuricemia. Results (1) The overall HUA detection rate was 18.70%, and HUA detection rate in male is higher than that in female (24% vsl 1.60%, P〈0.01). The detection rate of HUA increased progressively with age (/9〈0.01). The highest detection rate of HUA occurred to the group of 30 years old males (26.30%) and to the group of 70 years old (25.60%), respectively. (2) in the high uric acid group the levels of UA, BMI, total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C), FBG were higher than that in the normal uric acid group (P〈0.01), while the level of high density lipoprotein cholesterol (HDL-C) is lower than that in the normal uric acid group (P〈0.01). (3) The multiple-factor logistic regression analysis showed that the overall risk (OR) for HUA were 2.30 for high TG (95%CI: 2.05 - 2.57), 1.29 for high LDL-C (95%CI:1.13 - 1.46), 0.85 for high HDL-C (95%CI: 0.75 ~ 0.96), 2.52 for overweight (95%C1:2.27 - 2.79), and 1.73 for males (95%CI: 1.56 ~ 1.91). Conclusion The overall HUA detection rate was relatively high in Guilin. Overweight, high TG, high LDL-C and male are the independent risk factors of HUA, and high HDL-C maybe a protective factor.
