自拟益气滋肾汤治疗气阴两虚证狼疮性肾炎临床研究

摘　　要：目的:探寻气阴两虚证系统性红斑狼疮肾炎有效中医治疗处方,观察自拟方的临床疗效,研究其的作用靶点。方法:40例气阴两虚证系统性红斑狼疮肾炎患者随机分为对照组、自拟方高剂量组、中剂量组、低剂量组。四组病例分别给予泼尼松片1mg/kg、对照组加用六味地黄丸免煎剂治疗,治疗4周。治疗前、治疗后观察病例的临床症状积分和血清抗C1q抗体表达水平变化。结果:治疗4周后,自拟方高剂量组、中剂量组血清抗C1q抗体表达水平、临床积分明显低于对照组和低剂量组,具有统计学意义,(P≤0.05);而中剂量组观察指标最低。临床症状积分与血清抗C1q抗体表达水平具有一致性。结论:自拟方可减少系统性红斑狼疮肾炎临床积分,而中剂量组临床疗效最好,其作用机制可能与抑制血清抗C1q抗体表达有关。Objective： To explore the effective traditional Chinese medicine treatment prescription of Qi and Yin deficiency syndrome of lupus nephritis （LN） , to observe the clinical curative effect and explore the target of self-made decoction. Meth- ods： 40cases of Qi and Yin deficiency syndrome of LN were randomly divided into controlled group, high dose group, middle dose group and low dose group of Self-made Yiqi Zishen Decoction, all the patients of the four groups were given prednisone tab- lets 1 mg/kg, the controlled group was given Liuwei Dihuang Pills free decoction, patients were treated for 4weeks. When before and after treatment, observed the clinical integral, anti C1 q antibody of the patients. Results： 4weeks after the treatment, both clinical integral and anti Clq antibody of the high dose group, middle dose group were lower than the controlled group and low dose group respectively, with statistical significance, （P ≤0. 05）. While the results of middle dose group were the lowest of all. Conclusion： The patient of LN who was given Self-made Yiqi Zishen Decoction can reduce the clinical integral and anti Clq anti- body. The middle dose group acquired the best clinical curative effect of all groups. The mechanism related to the restrain of anti Clq antibody expression in serum probably.
