Cook子宫颈扩张球囊与欣普贝生用于足月妊娠引产的临床研究

摘　　要：目的：比较Cook子宫颈扩张球囊与欣普贝生促宫颈成熟用手足月妊娠引产的有效性和安全性。方法：选择2013年1月-2014年1月在深圳市第二人民医院符合引产指征用Cook子宫颈扩张球囊和欣普贝生促宫颈成熟引产孕妇各82例,采用对照研究方法观察两组促宫颈成熟有效性、分娩方式、分娩结局、新生儿结局及分娩并发症。结果：Cook子宫颈扩张球囊组引产有效率89.02%显著高于欣普贝生组的65.85%,阴道放置总时间（11.76±2.89）h较欣普贝生组的（8.30±5.96）h长,催产素总使用量（7.50±6.17）少于欣普贝生组的（8.54±1.20）,差异均有统计学意义（P〈0.05）;两组分娩方式、产后24 h出血量、新生儿1 min Apgar评分、羊水Ⅲ°污染、感染及宫颈裂伤情况差异均无统计学意义（P〉0.05）。结论：该两种引产方式都有一定的有效性和安全性,Cook子宫颈扩张球囊的有效性优于欣普贝生,安全性两者无明显差异。Objective： To compare the safety and efficacy of Cook cervical ripening balloon and dinoprostone for induction of term pregnancy. Methods： From January 2013 to January 2013, 82 pregnant women treated by Cook cervical ripening balloon and 82 pregnant women treated by dinoprostone were selected,a case- control study was performed to observe the efficacy,delivery modes,delivery outcomes,neonatal outcomes and pregnancy complications in the two groups. Results： The effective rate in Cook cervical ripening balloon group was 89. 02%,which was significantly higher than that in dinoprostone group（ 65. 85%）,the total application time in Cook cervical ripening balloon group was（ 11. 76 ± 2. 89） hours,which was significantly longer than that in dinoprostone group [（ 8. 30 ± 5. 96）hours],the total dose of oxytocin in Cook cervical ripening balloon group was（ 7. 50 ± 6. 17）,which was significantly less than that in dinoprostone group（ 8. 54 ± 1. 20）（ P 0. 05）; there was no statistically significant difference in the delivery mode,the amount of hemorrhage within 24 hours after delivery,neonatal one- minute Apgar score,the incidence rates of amniotic fluid pollution（ grade Ⅲ）,infection and cervical laceration between the two groups（ P 0. 05）. Conclusion： The two methods have a certain safety and efficacy. The effectiveness of Cook cervical ripening balloon is better than dinoprostone,but there is no significant difference in the safety between the two methods.
