2型糖尿病患者尿白蛋白水平相关因素的Logistic回归分析

摘　　要：目的研究影响2型糖尿病患者尿白蛋白水平的相关因素。方法选取浙江大学附属第一医院2012年9月至2013年9月收治的2型糖尿病患者90例,测定所有患者的超敏C反应蛋白(hs CRP)、空腹血糖(GLU)、糖化血红蛋白(Hb A1c)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、尿酸(UA)、维生素D(Vit D)、甲状旁腺激素(PTH)、肾小球滤过率(GFR)、尿白蛋白/尿肌酐比值(UACR)等指标。采用单因素以及多因素Logistic回归分析影响2型糖尿病UACR水平的相关因素。结果单因素Logistic回归显示hs CRP、Hb A1c、TG、UA、Vit D、GFR等因子与2型糖尿病患者UACR水平有关(P<0.05)。多因素Logistic回归分析显示Hb A1c、Vit D、GFR等因子与2型糖尿病患者UACR水平具有独立相关性(P<0.05)。结论Hb A1c、Vit D、GFR是影响2型糖尿病患者UACR水平的独立因素。Objective To investigate the related factors for urinary albumin levels in patients with type 2 diabetes mellitus.Methods A total of 90 cases of patients with type 2 diabetes were included as study subjects, the patients who were hospitalized in First Affiliated Hospital of Zhejiang University during September 2012 to September2013. The samples from these patients were measured for hs CRP, GLU, Hb A1 c, TG TC, HDL-C, UA, Vit D, PTH, GFR and UACR. The association between these related factors and albuminuria was evaluated by logistic regression analysis. Results Single factor logistic regression analysis showed that hs CRP, Hb A1 c, TG, UA, Vit D, GFR and other factors related to the level of UACR in patients with type 2 diabetes mellitus（P〈0.05）, however, multivariate logistic regression analysis showed that Hb A1 c, Vit D, GFR and other factors have independent correlation with UACR levels in patients with type 2 diabetes mellitus（P〈0.05）. Conclusion Hb A1 c, Vit D, GFR were the independent risk factors of albuminuria in patients with type 2 diabetes.
