摘  要:目的探讨类糜蛋白酶在药物过敏性休克死亡法医学诊断中的作用和意义。方法 10例死因确诊为药物过敏性休克的患者为研究组,排除过敏性休克、冠心病、海洛因中毒及颅脑损伤这4类疾病死亡者10例为对照组;应用免疫荧光染色法对上述两组肺组织进行类糜蛋白酶染色。Olympus BX61荧光显微镜观察并进行图像采集,Image pro图像分析系统对所采集图像进行分析。结果肺组织肥大细胞类糜蛋白酶阳性颗粒计数,研究组8.50±1.74,对照组0.95±0.42,研究组的表达较对照组明显升高(P<0.05);其表达量在死后48 h内无明显变化。结论肺组织中类糜蛋白酶的表达有望作为过敏性休克死亡法医学诊断的一种辅助指标。Objective To explore the role and significance of chymase in diagnosis of drug anaphylactic shock in the forensic medicine. Methods Ten cases of drug-related fatal anaphylaxis were collected as experiment group,and 10 cases dead from other diseases (except anaphylactic shock, coronary heart disease, heroinismus ,brain injury)were chosen as control group. The chymase expression in the lung tissues was observed by immunofluorescence staining. The collected images of immunoftuorescence staining were analyzed by Image-pro. Results The expression of chymase in experiment group was significantly higher than that in control group(8.50 ±1.74 vs 0. 95 ±0. 42,P 〈 0.05), and its expression had no obvious change within 48 h after death. Conclusion The expression of chymase in lung tissues can be a supplementary means for diagnosing drug-related fatal anaphylaxis in forensic medicine.
