2型糖尿病周围神经病变相关危险因素分析

摘　　要：目的分析2型糖尿病周围神经病变的相关危险因素。方法回顾性分析120例2型糖尿病患者,分为2型糖尿病合并周围神经病变组(DPN组)78例和2型糖尿病非合并周围神经病变组(DM组)42例,比较两组间的各项指标。结果DPN组与DM组单因素比较,DPN组的年龄、糖尿病病程、空腹血糖(FBG)、餐后2 h血糖(2h PG)、糖化血红蛋白(Hb A1c)及尿白蛋白与肌酐比值(ACR)较DM组均升高,DPN组的2 h胰岛素(2h Ins)明显低于DM组,差异均有统计学意义(P<0.05);而性别、体重指数(BMI)、收缩压(SBP)、舒张压(DBP)、空腹胰岛素(Ins)、甘油三脂(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)及高密度脂蛋白胆固醇(HDL-C)两组间比较差异均无统计学意义(P>0.05)。以是否DPN做应变量,以单因素比较有差异的指标做自变量,进行多因素Logistic多元回归性分析,结果显示糖尿病病程、Hb A1c、ACR为DPN的危险因素。结论 DPN的危险因素有糖尿病病程、Hb Alc、ACR。Objective To explore the risk factors of type 2 diabetic peripheral neuropathy. Methods A total of 120 cases of type 2 diabetes mellitus were enrolled in this study,including 42 cases of type 2 diabetes without peripheral neuropathy group （DM group）and 78 cases of type 2 diabetic peripheral neuropathy group（ DPN group）. The clinical data were compared between two groups. Results Compared with DM group, the age, duration of diabetes, fasting blood glucose （FBG）,2 h postprandial blood glucose（2hPG） , glyeosylated hemoglobin（HbAlc） and urinary albumin-to-creatinine ratio（ACR）were increased in DPN group, while 2 h insulin （2hIns）was significantly decreased（P 〈 0.05 ）. There was no significant difference in gender, body mass index （BMI）, systolic blood pressure （SBP） ,diastolic blood pressure （DBP）, fasting insulin, triglyceride （TG）, total cholesterol （TC）, low density lipoprotein cholesterol （LDL-C） and high density lipoprotein cholesterol （ HDL-C ） between the two groups （ P 〉 0.05 ）. The multi factor Logistic regression analysis showed that the diabetes duration, HbA1 c, ACR were risk factors of DPN. Conclusion Diabetes duration, HbAlc, ACR are the risk factors of DPN.
