宫腔镜联合B超在子宫内膜息肉诊断中的应用分析

摘　　要：目的探讨宫腔镜联合B超在子宫内膜息肉诊断中的应用价值。方法选取莱芜市人民医院2010年4月至2013年6月收治的因子宫异常出血就诊患者120例,分别采用B超检查、宫腔镜检查,对宫腔镜检查诊断为子宫内膜息肉的32例患者采取宫腔镜子宫内膜息肉电切术并进行病理检查,将术前B超及宫腔镜检查结果与术后病理结果进行比较。结果 B超检查、宫腔镜检查、宫腔镜联合B超检查的灵敏度分别为40.0%、90.0%、96.67%,特异度分别为91.11%、94.44%、97.78%,漏诊率分别为60.0%、10.0%、3.33%,误诊率分别为8.89%、5.56%、2.22%,Kappa值分别为0.674、0.867、0.934(P<0.05)。结论对子宫内膜息肉患者,术前宫腔镜检查较B超检查的准确率高,B超联合宫腔镜检查对术前诊断具有更高的临床价值。Objective To explore the application value of hysteroscopy combined with B ultrasound in diagnosis of endometrial polyps.Methods 120 patients admitted in Laiwu people’ s hospital、 hospital for abnormal uterine bleeding from 2010 April to 2013 June were selected , all cases were examined by B ultrasound and hysteroscopy , 32 cases were diagnosed as endometrial polyps by hysteroscopy were taken hysteroscopic endometrial polypectomy and pathological examination , the preoperative B ultrasound and hysteroscopy results were compared with pathological results .Results The sensitivity of B ultrasound , hysteroscopy , hysteroscopy combined with B ultrasound were 40.0 %, 90.0 %, 96.67 % respectively, the specificity of three methods were 91.11 %, 94.44%, 97.78 % respectively, the rate of missed diagnosis of three methods were 60.0 %, 10.0 %, 3.33 % respectively, the misdiagnosis rate of three methods were 8.89%, 5.56%, 2.22%respectively, the Kappa value of three methods were 0.674, 0.867, 0.934 respectively （ P〈0.05 ） .Conclusion For patients with endometrial polyps , the diagnostic accuracy rate of hysteroscopy is higher than B ultrasound , while hysteroscopy combined with B ultrasound has higher clinical value for preoperative diagnosis .
