戊酸雌二醇联合米索前列醇在绝经后妇女宫内节育器取出中的应用

摘　　要：目的探讨戊酸雌二醇联合米索前列醇在绝经后妇女宫内节育器(Intrauterine device,IUD)取出中的应用价值。方法对江阴市妇幼保健院2010年3月至2014年3月绝经多年后要求取出IUD的120例妇女临床资料进行回顾性分析。60例受术者术前7 d口服戊酸雌二醇,1次/d,1 mg/次,术前2 h阴道放置米索前列醇400 ug的为A组,60例受术者术前阴道2 h放置米索前列醇400 ug为B组。观察比较两组用药后宫颈软化程度、受术者疼痛程度、手术时间、取器成功率、术后出血时间及术后子宫内膜厚度。结果 A组60例均成功取器,B组中有12例取器失败,转为A组处理方法后均成功取器。A组在宫颈软化、受术者的疼痛程度、手术时间、取器成功率上明显优于B组(P﹤0.05)。术后出血时间及术后子宫内膜厚度两组间比较差异无统计学意义(P﹥0.05)。结论戊酸雌二醇联合米索前列醇用于绝经后妇女IUD取出,可减轻受术者痛苦,提高绝经后妇女取器成功率,安全有效。Objective To explore the application value of Progynova combined with Misoprostol using in removal of intrauterine device（IUD） for postmenopausal women, to improve the success rate of removal of IUD.Methods 120 women many years after menopause required removal of IUD in JiangYin Maternal and Child Health Hospital from Mar 2010 to Mar 2014 were selected , the data of all cases were retrospectively analyzed .All patients were divided into group A and group B with 60 case in each group , the cases in group A were given Progynova 7 days before operation , oral administration of 1 mg once daily ,400 ug Misoprostol were placed in the vaginal 2 hours before operation .The cases in group B were given 400 ug Misoprostol in the vaginal 2 hours before operation .The cervical softening, pain degree, operation time, success rate of removal of IUD, postoperative bleeding time and postoperative endometrial thickness of two groups were compared .Results 60 cases in group A were taken out IUD successfully ,while 12 cases in group B were failed.The cervical softening, pain degree, operation time, success rate of removal of IUD of group A were superior to that of group B , the differences were statistically significant （ p 〈0.05 ） .No significant differences were found in the postoperative＆amp;nbsp;bleeding time and postoperative endometrial thickness （p＆gt;0.05）.Conclusion Using progynova combined with misoprostol in removal of IUD for women after menopause can alleviate pain during operation , improve the successful rate of removal of IUD , and it is safe and effective.
