喘炎宁合剂对慢性阻塞性肺疾病急性加重期痰热蕴肺证患者的影响

摘　　要：目的:观察喘炎宁合剂对慢性阻塞性肺疾病急性加重期(acute exacerbationof chronic obstructive pulmonary disease,AECOPD)痰热蕴肺证患者的影响。方法:将65例AECOPD、中医辨证为痰热蕴肺证的患者分为治疗组33例,对照组32例。对照组采用常规疗法,治疗组在对照组的基础上给予喘炎宁合剂,疗程为10 d。观察两组患者治疗后症状改善情况、肺功能变化等。结果:治疗组患者咳嗽、咯痰、气喘等临床症状,与对照组比较,差异有统计学意义(P<0.05);与对照组比较,治疗组心率(86.03±10.24)次·min-1、呼吸频率(20.36±3.34)次·min-1、动脉二氧化碳分压(42.67±4.89)mm Hg(1 mm Hg=0.133k Pa)明显下降(P<0.05);治疗组Pa O2(75.76±11.56)mm Hg、Sa O2(92.61±2.96)%明显升高,与对照组相比,差异有统计学意义(P<0.01);治疗组肺功能指标用力肺活量(2.51±0.65)L、一秒钟用力呼气容积(1.62±0.55)L、最大呼气流速(3.02±0.76)L·s-1、一秒钟用力呼气容积百分比(58.90±11.10)%均明显改善,与对照组比较,差异有统计学意义(P<0.05或P<0.01)。结论:喘炎宁合剂能有效改善AECOPD痰热蕴肺证患者咳嗽、咯痰、气喘等临床症状,能显著改善患者肺通气功能、心率、呼吸频率等,且无明显不良反应。Objective：To observe the effect of acute exacerbation of chronic obstructive pulmonary disease（ AECOPD）with syndrome of retention of phlegm heat in the lung treated by Chuanyanning Mixture. Methods：65 cases of AECOPD with syndrome of retention of phlegm heat in the lung were divided into two groups,treatment group（33 cases）and control group（32 cases）. The control group was given conventional treatment,and the treatment group was treated with Chuanyanning Mixture based on the control group. 10 days as a course of treatment. Observed symptoms improvement after treatment and pulmonary function changes and others of the two groups. Re- sults：Chuanyanning Mixture could improve significantly clinical symptoms such as cough,phlegm and gasping and compared with the control group,the difference was significant（P﹤0. 05）,and in the treatment group,heart rate（HR）（86. 03 ± 10. 24）times per mi- nute,respiratory rate（RR）（20. 36 ± 3. 34）times per minute,arterial carbon dioxide partial pressure（42. 67 ± 4. 89）mmHg,compared with the control group,the difference were significant（P﹤0. 01）. In the treatment group,PaO2（75. 76 ± 11. 56）mmHg and SaO2 （92.61±2.96）%,comparedwiththecontrolgroup,thedifferenceweresignificant（P﹤0.01）.Lungfunctionindexforcedvitalca- pacity（2. 51 ± 0. 65）L,forced expiratory volume in one second（1. 62 ± 0. 55）L,maximum expiratory flow rate（3. 02 ± 0. 76）L·s-1,percentage of forced expiratory volume in one second（58. 90 ± 11. 10）%,compared with control group,the difference were statistically significant（P﹤0. 05 or P ﹤0. 01）. Conclusion：Chuanyanning mixture could improve clinical symptoms such as cough, phlegm and gasping obviously,raise the pulmonary ventilation function,RR and HR of AECOPD with syndrome of retention of phlegm heat in the lung and there is no significant side effects.
