椎旁神经阻滞治疗带状疱疹神经痛26例

摘　　要：目的探讨椎旁神经阻滞治疗带状疱疹神经痛的疗效。方法将46例带状疱疹后遗神经痛患者随机分为对照组20例和观察组26例。常规药物治疗组使用抗病毒、营养神经、止痛药等;神经阻滞组在药物治疗的基础上,在与病变区域相对应的神经支配节段于鞘内注射镇痛液行椎旁神经阻滞治疗。分别于治疗前,治疗后1周、2周、3周、4周采用疼痛视觉模拟评分(VAS评分),评估疼痛程度及不良反应等情况。结果两组治疗后VAS评分均明显降低,观察组治疗一周后VAS评分首先降低,对照组起效慢,两周后开始明显下降;治疗结束时两组镇痛总有效率均≥70%,但观察组疼痛完全缓解率(65.38%)及总有效率(84.61%)显著高于对照组。结论椎旁神经阻滞疗法对带状疱疹神经痛PHN临床症状的改善作用明显优于单纯药物治疗,且安全有效。Objective To study the near vertebral nerve block to treat the elderly efficacy and safety of heritage neu- ralgia after herpes zoster. Methods 46 patients with heritage neuralgia after herpes zoster were randomly divided into control group 20 cases and observation group of 26 patients. Conventional drug treatment group use antivirus, trophic nerve, painkillers, etc; Nerve block set on the basis of drug therapy, in the region of the lesions and the corresponding in- nervation segmental analgesia intrathecal injection liquid line near the vertebral nerve block therapy. Respectively before and after treatment 1 week, 2 weeks, 3 weeks, 4 weeks using visual analogue scale （VAS score）, pain assessment of pain degree and the adverse reactions, and so on and so forth. Results Two groups of VAS score were significantly lower af- ter treatment, the observation group 1 week after treatment first reduce VAS score, the control effect slow, two weeks after the start significantly decreased; Analgesic treatment at the end of the two groups total effective rate is 70% or higher, but the observation group complete pain relief rate （65.38%） and the total effective rate （84.61%）, significantly higher than the control group. Conclusion By the vertebral nerve block therapy for the elderly heritage neuralgia after herpes zoster PHN the improvement of clinical symptoms obviously better than pure drug therapy, and is safe and effective.
