川芎嗪对术中回收血的红细胞形态、ATP酶活性和携氧功能的影响

摘　　要：目的评价川芎嗪对术中回收血红细胞质量的保护作用。方法将连续46名择期拟行脊柱减压植骨融合内固定手术患者随机分成实验组(n=23):于回收血液前30 min静滴川芎嗪4 mg/(kg·人),并在用于抗凝的装有肝素生理盐水内加入川芎嗪,使其浓度为0.1 mg/m L;对照组(n=23):给予同实验组患者等容量的生理盐水。观察并比较2组回收红细胞的形态和异形红细胞的百分比,检测ATP酶活性、p H值和计算50%氧分压(P50)。结果实验组红细胞形态明显优于对照组;实验组与对照组异形红细胞占百分比(%)分别为19.2±4.4 vs 26.1±3.9(P<0.05),ATP酶活性(μmolpi·g Hb-1·h-1)分别为6.8±0.6 vs 6.1±0.4(P<0.05),P50值(mm Hg)分别为25.5±3.6vs 26.0±4.1(P>0.05)和p H分别为7.34±0.10 vs 7.35±0.08(P>0.05)。结论川芎嗪对术中回收红细胞的形态和ATP酶均具有保护作用,且能保持红细胞正常的携氧功能。Objective To evaluate the effects of ligustrazine on the protection of the quality of salvaged red blood cell. Methods Forty-six patients who underwent continuous elective spinal decompression and interbody fusion surgery were ran- domly divided into two groups. In treatment group （n = 23 ）, ligustrazine 4mg/kg had been injected intravenously to every patient 30 minutes before their blood was collected. Ligustrazine was added to the heparinized saline to achieve the final con- centration of 0. 1 mg/mL. Same volume of saline was added to the control group instead. Shed blood of both groups were col- lected, filtrated, separated and washed before re-infusion to the patients. The erythrocyte morphology of the salvaged blood was observed. The activity of ATPase, Ps0 and pH values were examined. Results Compared with the control group, the treatment group had superior erythrocyte morphology, with a lower percentage of non-disk shaped red ceils （ 19. 2 ± 4.4 vs 26. 1 -± 3.9, P 〈 0. 05 ） and a higher ATPase activity （ μmolpi · gHb-1·h-1）（6.8±0.6vs 6.1 ±0.4,P〈0.05）. There wereno significant differences in Ps0 （mmHg） （ 25.5± 3.6 vs 26. 0 ± 4. 1, P 〉 0. 05 ） and pH values （ 7. 34 ± 0. 10 vs 7. 35 ± 0. 08, P 〉 0. 05） between the two groups. Conclusion Ligustrazine can protect the shape of red blood cell and activity of ArPase in the salvaged red cells, and can maintain normal oxygen-carrying capacity.
