摘  要:目的通过对ABO血型不合异基因造血干细胞移植后患者血型延迟转化案例的分析,进一步明确此类患者血型延迟转化的机理和治疗手段。方法对患者移植术后(15~211 d)进行ABO血型连续监测;患者开始输注血浆治疗至血型完全转化为供者型期间,进行效价监测。结果患者于移植术后211 d血型完全转化为供者型;血清抗-A效价降至2、抗-B效价降至4。结论血清中存在的高效价抗A、抗B抗体导致供者红细胞遭到破坏是导致血型未转化的根本原因。通过血浆连续置换的治疗手段可取得良好效果。Objective To analyze the delayed blood type transformation caused by ABO incompatibility in patients re- ceiving allogeneic hematopoietic stem cell tranplantation; to further elucidate the mechanism that led to delayed transforma- tion in these patients ; to identify treatment. Methods A patient who has undergone transplantation surgeries was continu- ously monitored for 15 to 211 days on the delayed changes of her blood type. From the time of plasma infusion in patients to the time of complete transformation in blood type, the titer was determined. Results 211 days after transplantation, the pa- tient's blood type completely transformed. The titer of anti-A dropped to 2 and that of anti-B dropped to 4. Conclusion The primary reason for delayed blood type transformation could be attributed to the high titer of anti-A and anti-B in serum which might cause destruction of donor's red blood cells. Using continuous plasma replacement as treatment could be effective for blood type transformation.
