摘  要:目的了解基质金属蛋白酶9(MMP-9)基因-1562C/T多态性与广东汉族人群社区获得性肺炎(CAP)易患性的关系及其对MMP-9表达水平的影响。方法选择2012年5—12月中山大学附属第三医院门急诊和住院诊治的广东汉族CAP患者100例为病例组,以同期门诊体检的健康广东汉族体检者96例为对照组。采用酶联免疫吸附测定(ELISA)方法检测病例组发病期、治疗后与对照组的血清MMP-9水平,应用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)方法检测两组MMP-9基因-1562C/T的基因型及等位基因。结果病例组发病期、治疗后和对照组的血清MMP-9水平分别为(342.85±107.63)、(203.57±80.61)、(184.74±93.46)μg/L。病例组发病期MMP-9水平高于治疗后和对照组,差异有统计学意义(t配对=4.512,t=5.305,P<0.05);病例组治疗后MMP-9水平与对照组比较,差异无统计学意义(t=2.429,P=0.143)。酶切后产物2%琼脂糖凝胶电泳结果显示,病例组CC基因型73例、CT基因型27例;对照组CC基因型71例、CT基因型25例;两组均未检测到TT基因型。病例组与对照组基因型分布均符合Hardy-Weinberg平衡定律(P>0.05)。两组CC基因型、CT基因型的频率分布比较,差异无统计学意义(P>0.05);C等位基因和T等位基因的频率分布比较,差异无统计学意义(P>0.05)。病例组CC基因型与CT基因型患者发病期血清MMP-9水平,对照组CC基因型与CT基因型健康者血清MMP-9水平比较,差异均无统计学意义(P>0.05)。结论 MMP-9参与了CAP的炎性反应过程,MMP-9基因-1562C/T多态性与广东汉族人群CAP的易患性无关,且不影响MMP-9的表达水平。Objective To explore the relationship of matrix metalloproteinase-9( MMP-9) gene-1562C/T polymorphism to susceptibility of community-acquired pneumonia( CAP) and its influence on MMP-9 expression in Han nationality in Guangdong. Methods From May to December 2012,100 CAP Guangdong Han patients who visited or hospitalized in the Third Affiliated Hospital of Sun Yat-sen University were enrolled in study group,96 healthy Guangdong Han subjects in control group. Enzyme-linked immunosorbent assay( ELISA) was used to detect serum MMP-9 levels in study group at onset and after treatment and in control group. Polymerase chain reaction-restriction fragment length polymorphism( PCR-RDLP) was used to determine the genotypes and alleles of MMP-9 gene-1562 C / T. Results The serum MMP-9 levels of study group were( 342. 85 ± 107. 63) μg / L at onset,( 203. 57 ± 80. 61) μg / L after treatment,and that of control group was( 184. 74± 93. 46) μg / L,MMP-9 was higher in study group at onset than in study group after treatment and in control group( tpair=4. 512,t = 5. 305,P〈0. 05),and there was no difference between study group after treatment and control group( t = 2. 429,P = 0. 143). Post-digestion products 2% agarose gel electrophoresis results showed that there were 73 cases of CC genotype,27 of CT genotype in study group,71 cases of CC genotype,25 CT genotype in control group. No TT genotype was detected out.The genotype distribution of 2 groups accorded with Hardy-Weinberg equilibrium( P〉0. 05). There was no difference in the frequency distributions of genotypes CC,CT between 2 groups( P〉0. 05),no difference in frequency distributions of alleles C,T( P〉0. 05). In MMP-9 level,patients with CC genotype were not different from those with CT genotype in study group,subjects with CC genotype not different from those with CT genotype in control group( P〉0. 05).
