社区2型糖尿病路径化表单式管理效果评价

摘　　要：目的分析长期路径表单化管理模式在社区2型糖尿病管理中的干预效果。方法选取2011年7月—2012年7月安立社区2型糖尿病患者210例,遵循路径化管理流程和表单式项目管理内容,依据年度路径项目完成率分为第1组（年度路径项目完成率〉80%,141例）、第2组（年度路径项目完成率60%～80%,42例）、第3组（年度路径项目完成率〈60%,27例）。分别记录第1组、第2组、第3组干预后1年时年度干预项目整体达标率;比较3组干预后1年时医疗监测指标（空腹血糖、餐后2 h血糖、糖化血红蛋白）变化情况。结果第1组年度干预项目达标率为（90.2±2.1）%,第2组为（85.0±3.1）%,第3组为（75.0±3.3）%,3组间差异有统计学意义（F=435.77,P〈0.05）。3组干预后1年时空腹血糖、餐后2 h血糖、糖化血红蛋白比较,差异均有统计学意义（P〈0.05）;第2、3组干预后1年时空腹血糖、餐后2 h血糖、糖化血红蛋白均高于第1组（P〈0.05）。结论社区2型糖尿病路径化表单式管理明确了2型糖尿病医患双方的管理流程和相关内容,强化了患者依从性,提高了社区2型糖尿病整体管理、控制水平。Objective To analyze the effectiveness of long-term pathway intervention model for diabetes community management. Methods A total of 210 T2 DM patients from Anli Community from July 2011 to July 2012 were divided,according to the annual finishing rates of community comprehensive management schedules（ CCMS）,into groups Ⅰ（ 〉80%,n =141）,Ⅱ（ 60%-80%,n = 42）,Ⅲ（ 〈60%,n = 27）. The annual overall finishing rates were recorded and the changes of medical monitoring indexes 〔fasting blood glucose（ FBG）,2 h postprandial blood glucose（ 2 h PBG）,glycosylated hemoglobin（ Hb A1c） 〕compared among 3 group 1 year after intervention. Results The annual finishing rate was（ 90. 2 ± 2. 1） % in group Ⅰ,（ 85. 0 ± 3. 1） % in group Ⅱ,（ 75. 0 ± 3. 3） % in group Ⅲ,the difference was significant（ F = 435. 77,P〈0. 05）. There was difference in FBG,2 h PBG,Hb A1 camong 3 groups 1 year after intervention（ P〈0. 05）,and higher in groups Ⅱ,Ⅲ than in group Ⅰ（ P〈0. 05）. Conclusion CCMS defining the process and content of T2 DM management can improve patients＇ compliance and the overall level of T2 DM community management and control.
