脑室镜联合脑室外引流术治疗原发性脑室出血的疗效分析

摘　　要：目的:探讨单纯脑室外引流术及脑室镜联合脑室外引流术2种手术方法治疗原发性脑室出血的治疗效果。方法:将原发性脑室出血68例患者用随机数字表的方法随机分成两组,分别采取单纯脑室外引流术(对照组)及脑室镜联合脑室引流术(治疗组)的手术方法进行治疗。结果:治疗组中,2例死亡,死亡率为5.88%,对照组中,死亡5例,死亡率为14.71%,差异有统计学意义(P<0.03)。在拔管时间、再出血率、住院时间、术后30 d脑积水发生率方面比较P=0.02、0.03、0.02、0.04,差异有统计学意义。结论:脑室镜联合脑室外引流术治疗原发性脑室出血患者,可以降低死亡率、缩短拔管时间、缩短住院时间、降低再出血率及降低远期脑积水发生率,因此脑室镜联合脑室外引流术的治疗方法是安全、有效的。Objective To investigate the external ventricular drainage and surgery combined with external ventricular drainage in the treatment of 2 kinds of operation method of primary intraventricular hemorrhage. Methods Primary intravetatricular hemorrhage in 68 patients with the method of random number table were randomly divided into two groups, respectively take simple ventricle external drainage （control group） drainage and surgery combined with ventricle （ treatment group） were treated by operation. Results In the treatment group,2 cases died,the mortality rate was 5.88%, the control group,5 cases died, the mortality rate was 14. 71%, P value 0. 03, time of tube, rebleeding rate, hospitalization time, postoperative 30 day incidence of hydrocephalus on P values were 0. 02,0. 03,0. 02,0. 04 in the drawing,there was significance. Conclusion Surgery combined with external ventricular drainage in the treatment of primary intraventricular hemorrhage, can reduce the mortality rate, shorten the time of extubation, shorten the hospitalization time ~ reduce the rate of rebleeding and decrease the incidence of hydrocephalus by ventriculoscope, so combined with external ventricular drainage treatment surgery is safe and effective.
