房间隔缺损合并肺动脉高压患儿导管封堵术后左房结构及功能的变化

摘　　要：目的:利用超声心动图评价儿童房间隔缺损(ASD)合并肺动脉高压(PAH)患者导管封堵术后左房大小和功能的变化。方法:选择成功施行封堵治疗的儿童继发孔型ASD合并PAH患者112例和于治疗前和治疗后1d、3个月、6个月、1年行经胸超声心动图检查,测量左房前后径及左房相关容积。心导管测肺动脉压及分流量。结果:治疗后1d,左房前后径与治疗前比较差异无统计学意义(P>0.05),治疗后3个月、6个月、1年左房前后径减小,与治疗前比较差异有统计学意义(P<0.05);治疗后1d、3个月、6个月、1年,肺动脉压力、左房最大、最小、收缩前容积较治疗前有明显差异(P<0.05);治疗后1d,左房整体射血分数、左房被动射血分数、左房主动射血分数与治疗前比较差异无统计学意义(P>0.05);治疗后3个月、6个月、1年,与治疗前比较差异均有统计学意义(P<0.05)。结论:儿童继发孔型ASD合并PAH患者封堵治疗后左房形态及功能明显改善。Objective：To discuss the changes of the left atria size and function occurring after transcatheter closure for atrial septal defect（ASD）associated with pulmonary arterial hypertension（PAH）children patients.Method：One hundred and twelve cases with ASD and PAH received iranscatheter elostlre successfully.Transthoracic echoeardiogram examination was performed to detect the diameter and volume of left atrium before treatment and in 1day,3,6months,1year after treatment.The pumomary artery pressure and shunted blood were determined by cardiac catheterization.Result：Left atrial diameter in 1day after treatment compared with that before treatment had no statistical significance（P〉0.05）,and the LAD was shorter at 3months,6months,1year after treatment than that before treatment（P〈0.05）;PAP,LAVmax,LAVmin and LAVpre in 1day,3months,6months,1year after treatment compared with that before treatment had statistical significance（P 0.05）;LATEF,LAPEF and LAAEF in 1day after treatment compared with that before treatment had no statistical significance（P〉0.05）,and the data in 3 months,6 months,1year after treatment was shorter than that before treatment（P〈0.05）.Conclusion：The formation and function of left atrium of children with ASD and PAH would be significantly improved after transcatheter closure.
